超声内镜在胃癌术前诊断和分期中的作用

Role of Endoscopic Ultrasonography in the Diagnosis and Pre - operative Staging of Gastric Carcinoma

目的 评估超声内镜在胃癌诊断和术前分期中的作用。方法 94例患者于术前行超声内镜和胃镜检查,并将其分期结果与术后病理学检查分期结果比较。结果 胃镜检查活检病理诊断率94.7％(89/94),超声内镜诊断率为91.5％(86/94)。胃镜联合超声内镜诊断率为100％。超声内镜分期结果与病理学分期比较,各期准确率分别为90％(T_1,9/10),85.2％(T_2,23/27),88.2％(T_3,30/34),91.3％(T_4,21/23);淋巴结转移准确率为79.8％。结论 超声内镜可有效协助诊断胃癌,并进行术前分期,有助于制订合理的手术方案。

Objective To evaluate the role of endoscopic ultrasonography in the diagnosis and pre - operative staging of gastric carcinoma. Methods Ninety - four patients were performed with endos-copy and endoscopic ultrasonography. And the staging results of endoscopic ultrasonography were compared to that of post - operative pathological assessment. Results The diagnostic accuracy is of endoscopic biopsy 94.7% (89/94) ,and that of endoscopic ultrasonography is 91. 5% (86/94) , while the concordance was 100% when two procedures were put together. Compared with the pathological staging, the endoscopic ultrasonography staging accuracy is 90% ( T, ,9/10) ,85. 2% ( T2,23/27 ) ,88. 2% ( T., ,30/34) and 91. 3% ( T4,21/23) respectively. And lymph node metastasis accuracy is 79.8%. Conclusion endoscopic ultrasonography is effective in diagnosing and pre - operative staging of gastric carcinoma.
