癸水清对肾功能衰竭大鼠的影响

摘要 目的 :探讨癸水清对实验性慢性肾功能衰竭 (CRF)的作用。方法 :以 5 /6肾切除的方法复制CRF大鼠模型 ,于实验第 16周末进行 2 4h尿蛋白定量 (Uprot) ,尿 β2 微球蛋白 ( β2 MG) ,肾功能 ,血脂 ,血清白蛋白 (Alb) ,血浆内皮素 (ET)含量的测定 ,取肾组织进行光镜下的病理形态学观察。结果 :与模型组及尿毒清组相比 ,癸水清使CRF大鼠Ccr显著升高、Scr、BUN显著降低、Up rot、尿 β2 MG显著减少 ;甘油三酯 (TG)、总胆固醇 (TC)、低密度脂蛋白胆固醇 (LDL ch)、血浆内皮素 (ET)显著降低 ;高密度脂蛋白胆固醇 (HDL ch)、血清Alb显著增高。CRF大鼠肾病理形态学明显改善。结论 :癸水清能减少尿蛋白的丢失 ,提高内生肌酐清除率 ,纠正脂质代谢紊乱 ,改善负氮平衡 ,降低血浆内皮素 ,并促进肾组织病理损害的修复 ,从而延缓CRF的恶化。 Objective: To investigate the effect of Guishuiqing on chronic renal failure in rats by 5/6 nephrectomy. Methods: forty-eight male SD rats were selected and randomly divided into four groups including model group, Niaoduqing (尿毒清) group, Guishuiqing group and sham operation group. 5/6 nephrectomy was adopted in each group to duplicate CRF model, except for sham group. By the end of the sixteenth weeks, urinary creatinine (Ucr), 24-hour’s urine protein quantitative analysis(Uprot) and urinary β 2 microglobulin (β 2-MG) were examined. We also observed the content of serum creatinine(Scr), blood uria nitrogen(BUN), serum albumin(ALB), total cholesterol(TC), triglyceride(TG), high density lipoprotein- cholesterol (HDL-ch) and low density lipoprotein-cholesterol(LDL-ch). The pathomorphology changes of kidney tissue were assessed by light and electron microscope. Results: ①In Guishuiqing group, the Ccr of rats with CRF was larger than that in model group and Niaoduqing group, while Scr and BUN obviously decreased. ②Guishuiqing could also decrease urine protein and β 2-MG. ③Guishuiqing could down-regulated triglyceride(TG) , total cholesterol(TC) and low density lipoprotein-cholesterol (LDL-ch), while up-regulating high density lipoprotein-cholesterol (HDL-ch) and Alb. ④The levels of plasma endothelin were lower in Guishuiqing group.⑤The pathologic damages of kidney tissue in Guishuiqing group were improved than that in model group and Niaoduqing group. (P<0.05 or P<0.01). Conclusion: It suggest that Guishuiqing can promote renal function, redress the disorder of lipid metabolism, elevate the content of plasma protein, rectify the negative nitrogen balance and decrease the content of ET, which play a important role in prolong the progress of CRF.
