脑梗死患者血清可溶性细胞间黏附分子1的变化及临床意义 被引量:3

摘要 目的 探讨急性脑梗死患者血清可溶性细胞间黏附分子 1(intercellularadhesionmolecule 1,sICAM 1)的变化及临床意义。方法 采用双抗体夹心ELISA法测定 5 3例急性脑梗死患者血清sICAM 1水平 ,并与 32例脑动脉硬化和 36例健康人对照比较。结果 脑梗死患者血清sICAM 1水平在脑梗死发生第 1天明显升高 ,高于脑动脉硬化及正常对照组。 1天至 3天呈上升趋势 ,3天至 7天呈下降趋势 ,14天基本降至正常水平。脑梗死患者组病情发作后 1、3、7天sICAM 1水平与脑动脉硬化及健康对照组比较显著升高。梗死体积越大、神经功能缺损程度越重 ,其水平越高。结论 sICAM 1与急性脑梗死密切相关 ,参与了脑梗死缺血性损伤的发生、发展过程。血清sICAM 1水平在一定程度上反映了脑梗死灶的大小和神经功能缺损程度 ,可作为了解脑梗死病情及变化的指标。 Objective To investigate the change of solubal intercellular adhesion molecule-1(sICAM-1) in patients with acute cerebral infarction and its clinical significance.Methods The level of sICAM-1 was measured serially with two-layer antibody sandwich enzyme linked immunosorbent assay(ELISA) in 53 patients with acute cerebral infarction after onset, 32 patients with cerebal atherosclerosis(CAS) and 36 normal controls. Results The level of sICAM-1 in patients with cerebral infarction increased from 1 day after onset to 3 days and decreased from after 3 days to 7 days, and returned almost to the level of normal controls in 14 days. The level of sICAM-1 in patients with cerebral infarction 1 day,3 days,7 days after the onset was significantly higher than that in patients with CAS and normal controls (P< 0.01). The level of sICAM-1 in patients with cerebral infarction 3 days after onset was significantly different between the patients with large and medium as well small cerebral infarction (P< 0.01).sICAM-1 level increased with the infarction volumn and neurological function deficit score.Conclusion sICAM-1 is closely related to the development of acute cerebral infarction and plays an important role in pathological procedure of cerebral damage after cerebral infarction. The level of sICAM-1 in patients with cerebral infarction implies the size of their cerebral infarction and degree of their neurological function deficit scores.
