胰蛋白酶抑制剂对脓毒症大鼠肠黏膜屏障的保护作用 被引量:13

摘要 目的探讨胰蛋白酶抑制剂乌司他丁对脓毒症大鼠肠黏膜屏障的保护作用。方法将 2 2只SD大鼠随机分为 3组 :假手术组、脓毒症组及干预组。利用盲肠结扎打孔法 (cecalligationandpunture ,CLP)制作大鼠脓毒症模型 ,干预组分别于CLP后 5h、15h腹腔给予 7 5× 10 4U/kg体重的乌司他丁 (ulinastatin ,UTI)。应用荧光光谱分析仪检测至肠腔进入门静脉的荧光标记葡聚糖(FITC D)的浓度 ,了解UTI对肠黏膜通透性的影响。同时 ,用透射电镜观察肠黏膜机械屏障的变化。结果CLP 2 1h后干预组动物脓毒症表现轻 ,假手术组门静脉FITC D浓度 (1 2 2± 0 2 1) μg/ml,而脓毒症组 (2 5 1± 0 5 6 ) μg/ml高于假手术组 (F =5 0 31,P <0 0 1) ,干预组 (1 5 7± 0 5 0 ) μg/ml则低于脓毒症组 (F =1 2 4 9,P <0 0 1)。脓毒症组肠黏膜机械屏障明显受损 ,而干预组肠屏障受损较轻。结论给予乌司他丁对脓毒症大鼠肠黏膜屏障有明显的保护作用。 Objective To investigate the protective role of Ulinastatin UTI on gut barrier of septic rats. Methods Twenty-two SD rats were divided into three groups: sham laparatomy(S), cecal ligation and punture(CLP), and CLP plus UTI. Septic rat model was estabilished through CLP method. Fluorescence spectrometry was used to measure FITC-dextran concentration from bowel lumen to portal vein. Ultrastructure of intestinal mucosa was observed under transmission eletron microscope (TEM). Results Twenty-one hrs after CLP septic symptoms in CLP plus UTI group were milder than those in CLP group. Portal concentration of FITC-dextran in CLP group was higher than that of S group[S (1.22±0.21) μg/ml vs. CLP (2.51±0.56) μg/ml, P<0.01], while in CLP plus UTI group it was decreased (1.57±0.50) μg/ml , P<0.01. TEM observation showed that UTI protected the ultrastructure of intetinal mucosa in septic rats. ConclusionUTI can significantly protect gut barrier in septic rats.
