浅析腹腔镜前列腺根治性切除术后尿管滑脱原因及处置策略

Countermeasures and reasons of catheter slippage after laparoscopic radical prostatectomy

目的:探讨腹腔镜前列腺根治性切除术后尿管滑脱原因与处理措施。方法:回顾性分析中国人民解放军总医院2016年2~6月行腹腔镜前列腺根治性切除术后5例尿管滑脱临床资料及术后处理情况,观察重置尿管后术后恢复情况。结果:1例经床旁二次留置尿管成功,1例经输尿管镜下完成二次置管,3例在电子膀胱镜下完成二次留置尿管。结论:术前详细检查尿管,术中精良操作,术后尿管护理轻柔操作可降低尿管滑脱风险,电子膀胱镜下二次留置尿管是安全、有效的方法,值得临床应用。

Objective:To investigate the countermeasures and reasons of catheter slippage after laparoscopic radical prostatectomy(LRP).Methods:The clinical data of 5cases of catheter slippage after LRP during February2016 to June 2016 in Chinese PLA General Hospital were retrospectively analyzed.The postoperative recovery of the patients after catheter replacement was observed.Results:One case was successfully treated by replacing urinary catheter.One case was treated by ureteroscope successfully.The catheter was replaced in 3cases under electron flexible cystoscope.Conclusions:The variety of measures can reduce the risk of catheter slippage,including the detailed examination of urinary catheter before operation,subtle manipulation,and the urinary catheter nursing carefully after operation.The replacement of the catheter under electron flexible cystoscope is safe,effective,and worthy of clinical promotion.
