视神经管内结构的显微解剖学研究

Microsurgical Anatomy of The Interacandicular Structure of the optic Canal

目的 为视神经减压术和进一步探索视神经间接损伤的病理机制提供形态学依据。方法 在手术显微镜下对 18例 (3 6侧 )成人颅底湿标本视神经管进行了解剖观测 ,测量了管内段视神经的口径、视神经硬膜鞘各壁的厚度、眼动脉与视神经的位置关系以及眼动脉与视神经管下壁的关系。结果 ①从视神经管颅口到眶口硬膜鞘的各壁逐渐增厚 ;②在视神经管颅口处 ,61 2 %眼动脉位于视神经的内下方 ,到了眶口处 ,81 9%眼动脉绕到视神经的外下方 ;③眼动脉在视神经管内行走于视神经硬膜鞘下壁内。所得数据用SPSS10 0软件进行统计学处理。结论 视神经间接损伤后 ,硬膜鞘在眶口处对视神经的束缚作用最强。由于管内段视神经被牢固地定于视神经管内 ,在外力作用下 ,管内段视神经能承受位移的潜力有限。

Objective To provide detailed morphological knowl ed ge for optic nerve decompression and farther understanding on the pathologic mec hanisms of indirect optic nerve injury.Methods Thirty six optic canals in the eighteen fixed hum an cadaver heads were observed and measured under the operating microscope.The c alibers of intracanalicular optic nerve were measured,the thickness of dura were measured,the detailed anatomie relation between the opthalmic artery and optic nerve was observed and the anatomie relation between the opthalmic artery and op tic nerve was observed and the anatomie relation between the ophthalmic artery a nd the inferior wall of optic canal was observed.All the data were analysed by S PSS software.Results ① From the cranial end to orbital end of optic canal the thickness of the dura gradually increases.② At the cranial e nd,61 2% ophthalmic arterys were located on the inferior medial side of the opt ic nerve and 81 9% opthalmic arterys rotated to the inferior lateral side at th e orbital end.③ The ophthalmic artery was located on the inferior wall of dura. ConclusionIn the orbital end,the restraining of the dura s heath on the injured optic nerve may be the most powerful.Due to the optic nerve was firmly fixed in the optie canal,the potential for the optic nerve's moving was limited while being injuried.
