Elementary Analysis on the Implementation Effects of Case-Based per Diem Payment Reform under New Rural Cooperative Medical System

目的:分析试点医院在新农合下实施按病种床日付费方式改革的效果及关键影响因素,为进一步完善改革方案提供参考和依据。方法:运用归纳法对定性资料进行分析,运用SPSS 19.0软件对改革前后的定量数据进行数理统计分析。结果:政策层面大力支持改革,但缺乏有效监管;改革方案在一定程度上起到引导试点医院降低平均住院天数和次均费用的作用;试点医院积极配合改革工作。结论:在政策层面要加强监管力度,改革方案仍需进一步完善;试点医院要不断加强自我管理。

Objective:To analyze the effect and the key influencing factors of case-based per diem payment reform under the new rural cooperative medical system so as to provide references for further implement the reform scheme.Methods:Induction was employed to analyze the qualitative data. SPSS19.0 Software was applied to perform mathematical statistic analysis on quantitative data before and after the reform.Results:Reforms needed the strong support from policy level, but lacking of effective supervision; to a certain extent, reform scheme lead hospitals to lower the average hospitalization days and average cost per visit; the pilot hospitals actively cooperated with reform.Conclusion:The power of supervision in policy level needed to be strengthened. Reform remained to be further improved. Pilot hospitals needed to strengthen the self management.
