摘要 目的 探讨鼻腔内翻性乳头状瘤组织中Ki-67抗原和增殖细胞核抗原(PCNA)的表达及差异,以寻找能较好准确灵敏表达鼻腔内翻性乳头状瘤增殖活性的指标。方法 应用免疫组化S-P法检测30例鼻腔内翻性乳头状瘤、15例鼻息肉及10例鼻腔鳞状细胞癌组织中Ki-67和PCNA的表达。显微镜下(400×)分别计数Ki-67、增殖细胞核抗原(PCNA)阳性细胞数占计数细胞总数的百分比,作为Ki-67、PCNA阳性指数(Ki-67-LI和PCNA-LI)。结果 鼻息肉、鼻腔内翻性乳头状瘤及鼻腔鳞癌组织中Ki-67阳性指数及PCNA阳性指数分别为:2.3±2.1和5.4±5.2;11.3±4.1和29.9±10.2;35.1±6.9和62.8±17.7,统计分析表明差异均有显著性(P<0.01)。结论 Ki-67-LI和PCNA-LI与肿瘤病理类型有很好的一致性,二者可分别作为反映鼻腔内翻性乳头状瘤增殖活性的指标。 Objective To determine the expression of Ki-67 antigen and proliferating cell nuclear antigen (PCNA) in nasal polyps (NP), nasal inverted papilloma(NIP) and nasal squamous cell carcinoma (NSCC) and to analyze the difference between them so as to find a strong independent indicator to judge the proliferative activity of NIP.Methods The expression of Ki-67 antigen and proliferating cell nuclear antigen were examined by immuno-histochemical technique ( S-P method) in 30 cases of nasal inverted papilloma, 15 cases of nasal polyps and 10 cases of nasal squamous cell carcinoma. The count of Ki-67 (PCNA)-positive cells was done by taking into account only the nuclear-stained cells in 4 fields (400 x ) within the areas having the highest labeling rates among the 1 000 tumor cells . The positively and negatively stained nuclei were noted, and the final counts were recorded in percentage. Results The expression rate of Ki-67 protein (Ki-67-11) was 2.3±2.1, 5.4±5.2 and 11.3 ± 4.1 respectively in nasal polyps, nasal inverted papilloma and nasal squamous cell carcinoma and a statistically significant difference in Ki-67 protein expression(Ki-67-LI) was found among these three diseases(P < 0.01). The expression rate of PCNA(PCNA-LI) was29.9± 10.2,35.1 ± 6.9 and 62.8± 17.7 respectively in nasal polyps, nasal inverted papilloma and nasal squamous cell carcinoma and a statistically significant difference in PCNA expression (PCNA-LI) was also found among these three diseases( P < 0.01) . Conclusion There are significant relations between Ki-67-LI, PCNA-LI and pathological types of nasal neoplasms. Both Ki-67-LI and PCNA-LI can be used as independent indicators to judge the proliferative activity of nasal inverted papilloma and in comparison with PCNA-LI, Ki-67-LI is better.
