腓肠神经营养血管蒂皮瓣修复胫前组织缺损 被引量:6

摘要 目的探讨一种创伤性胫前组织缺损的修复方法,特别是伴有骨骼裸露的软组织缺损的覆盖问题。方法依据胫前组织缺损的大小、形态和部位的不同,采取腓肠神经营养血管蒂皮瓣顺行或逆行转移的方法修复,在蒂部设计三角形皮瓣改善逆行皮瓣的静脉回流。结果皮瓣面积最小5cm×4cm,最大13cm×8cm,全部成活。结论腓肠神经营养血管蒂皮瓣应用于胫前组织缺损的修复,安全可靠、成活率高,只要供区条件允许可作为首选方案。 Objective To explore the clinical methods for repairing anterior tibia tissue defect,especially for repairing bone tissue exposed.Methods According to the area,shape and positionof anterior tibia tissue defect,we applied anterograde and retrograde transferation,and designed a method for reducing the pressue of subcutaneous turnel to vessel pedicel,threrfore it improved the venous return of flap.Results The flap was used in total14cases with a maximum size of 13cm×8cm and minimum size of 5cm×4cm,and all flaps survived completely.Conclusion The sural neurovascular flap has high survival rate and sufficient safety for repairing anterior tibia tissue defect without vascular anastomosis and major artery injury of lower limb,it is the first measure to be selected,with the scope available.
