血清高分子量脂联素多聚体水平检测在慢性肾衰患者诊疗中的意义

摘要 目的探讨血清高分子量脂联素多聚体水平检测在慢性患者诊疗中的临床价值。方法将该院2013年7月—2014年7月收治的80例慢性衰竭患者随机分成A、B两组,各40例,其中A组为非血液透析的患者、B组为血液透析的患者,然后取38例无慢性肾衰的健康人作为C组。比较三组检测后的临床效果。结果检测后,A组和B组的总APN水平、血清HMW-APN水平等均高于C组;B组粥样斑块发生率、IMT厚度等指标均高于A组,且差异有统计学意义(P<0.05);B组血清HMW-APN水平明显低于A组,且差异有统计学意义(P<0.05);在检测过程中,血清HMWAPN水平与尿氮素、肌酐成正相关,与甘油三酯呈负相关。另外,总APN水平与肌酐呈正相关。结论血清高分子量脂联素多聚体水平检测在慢性肾衰患者的诊疗中发挥着重要作用,并且同时检测血清HMP-APN与总APN水平能有效成为检测慢性肾衰的关键指标,以及较好的反映抗动脉硬化等并发症的产生,值得推广。 Objective To investigate the serum high molecular weight adiponectin multimers Levels clinical value in the diagnosis and treatment of patients with chronic. Methods 80 patients with chronic failure patients admitted to our hospital from July 2013 to July 2014 were randomly divided into A, B groups, with 40 cases, of which A group of non-hemodialysis patients, hemodialysis patients in group B, and then take the 38 cases without chronic renal failure healthy people as a group C. Comparison of the three groups after the detection of clinical effect. Results After testing, the overall level of APN group A and group B, serum HMW-APN levels are higher than group C; group B plaque incidence, IMT thickness and other indicators were higher than the A group, and the difference was stat istically show significance(P<0.05); serum HMW-APN group B was significantly lower than that in group A, and show a statistically significant difference(P<0.05); in the detection process, serum HMW-APN levels and urinary nitrogen, creatinine positively correlated negatively correlated with triglycerides. In addition, the total APN creatinine levels were positively correlated. Conclusions The serum high molecular weight adiponectin multimers level detection plays an important role in the diagnosis and treatment of patients with chronic renal failure, and simultaneous detection of serum total HMP-APN APN level detection can effectively become a key indicator of chronic renal failure, as well as better produced response against atherosclerosis and other complications, it is worth promoting.
