摘要 目的 研究胃癌细胞凋亡和细胞增殖状态的变化以及p5 3基因蛋白表达 ,探讨其在胃癌恶性转化进程中的作用。方法 采用脱氧核糖核酸转移酶介导的生物素化dUTP缺口末端标记(TUNEL)技术和免疫组织化学S -P法分别检测 5 8例胃癌及癌旁粘膜组织凋亡和增殖状态及 p5 3基因的表达。结果 胃癌组织细胞凋亡指数显著低于癌旁对照组织 (2 .8± 0 .8、6 .9± 0 .3,P <0 .0 1) ;前者细胞增殖指数明显高于后者 (6 5± 16 .2、11± 1.0 1、P<0 .0 1) ,且凋亡 /增殖比率呈递减趋势 (0 .0 5±0 .0 1、0 .6 4± 0 .0 3,P <0 .0 1)。胃癌p5 3基因突变阳性组与阴性组的凋亡指数AI分别为 1.6 1± 0 .4和4 .15± 0 .7;PI分别为 72± 6 .5、5 4± 8.1,差异均有显著性 (P<0 .0 1)。结论 在胃癌组织中癌细胞凋亡明显受到抑制 ,细胞凋亡、增殖状态和 p5 3基因表达的改变是胃癌发生的重要因素之一。 Objective To investigate the changes and the effects of the cell apoptosis, proliferation and expression of p53 protein in gastric carcinoma. Methods The methods of TUNEL, immunohistochemical S-P were used to assay apoptotic index (AI) and proliferating index (PI) and expression of p53 protein in 58 cases of gastric carcinoma. Results The apoptotic indexes in gastric carcinoma were significantly lower than that in normal mucosa(2.8、6.9, P< 0.01). The proliferating indexes in gastric carcinoma were significantly higher than those in normal mucosa(65、11, P< 0.01) and the ratio of apoptosis to that of proliferation decreased. Comparative rate of p53 expression positive and negative,the AI was 1.61, 4.15 ( P< 0.01), the PI was 72, 54 ( P< 0.01). Conclusions Apoptosis is inhibited in gastric carcinoma. Abnormality in cell apoptosis and proliferation, changes expression of p53 protein implies important significance for the genesis of gastric carcinoma.
