腹水直接回输治疗难治性腹水患者的护理体会

Experience of nursing patients with difficultly curing ascites treated by directly returning ascites

目的探讨血液透析中难治性腹水的治疗方法与护理。方法对1998年6月～2002年3月25例难治性腹水的病人,在血液透析中进行105例次直接自身腹水回输。结果病人的腹围明显缩小,体质量下降,血浆白蛋白明显升高,取得了满意的治疗护理效果。结论血液透析中自身腹水直接回输法具有治疗成本低,能有效提高血浆白蛋白,感染机会少,操作方便、简单,安全的优点,是一种治疗难治性腹水的好方法。在腹水回输的护理中,要严格执行无菌操作,准确调节好超滤量,保持出入平衡,预防并发症的发生,是保证腹水回输顺利的关键。

Objective To explore the treatment and nursing of patients with difficultly curing ascites at hemodialysis. Methods 25 patients admitted from June 1998 to March 2002 were treated by directly returning ascites at hemodialysis for 105 case-times. Results Their abdomen circumference were markedly shortened, weight went down, serum albumin went up. The therapeutic efficacy and nursing were satisfactory. Conclusion Directly returning ascites at hemodialysis is low at cost and it can effectively increase serum albumin so that infections become fewer. Simple and safe at operation, it is a good therapy in treating difficultly curing ascites. In the process of returning the ascites, strict sterile operation, exact adjustment of ultrafitration volume, keeping in-and-out balance and prevention of complications are key to the success in returning the ascites.
