Biomechanical Properties of Skin Expanded by Different Methods in vivo

摘要 成形外科中 ,人们希望用快速扩张来代替常规扩张 ,因为这样可以大大缩短扩张时间。然而快速扩张后的皮肤特性是否接近于常规扩张 ,这是人们所关切的问题。据此我们设计和制成了在体张力量测仪并首次进行在体生物力学试验研究。试验表明在扩张结束后 ,扩张面积的测定表明 ,扩张皮瓣中心面积增加和整个扩张面积的增加在各种扩张方案下的差别不大 ;皮肤的在体张力和即时回缩在扩张时都有明显的增加 ,然而 ,随着维持时间的增加 ,慢慢向正常水平恢复。因此我们得出结论 ,快速扩张组的生物力学特性十分接近于常规扩张组 ,而且延长维持时间可以提高皮肤的生物力学性质。据此 ,临床上可以采取缩短扩张期 。 In plastic surgery, people hope the conventional expansion will be replaced by rapid expansion, which can shorten greatly the expansion period. Also people are concerned about whether the skin properties after rapid expansion approach that after conventional expansion. So we designed and made an apparatus for in vivo measurement of tension, and then measured the biomechanical properties of skin in vivo for the first time. The experimental results showed there was no significant difference between the rapid expansion method and conventional expansion method in respect to the area gain of expanded skin surface. Both the tension in vivo and the instant stretch back ratio increased during the expansion, but fell almost to the control values after four weeks' maintaining period. So, rapid skin expansion did not produce any deleterious effect when compared with the conventional expansion. Extension of the maintaining period can improve the biomechanical properties of expanded skin and effectively reduce the stretch back ratio. Therefore, rapid expansion with an extended maintaining period is clinically acceptable.
