高血压与冠心病患者左室舒张功能的比较

Compared with left ventricular diastolic function in hypertension and coro nary artery diseases

目的:探讨高血压与冠心病患者左室舒张功能的特点。方法:利用彩超测量冠心病组、高血压组(左室肥厚组及非左室肥厚组)及正常对照组的的二尖瓣血流E峰、A峰、E/A值、等容舒张时间(IVRT)、E峰减速时间(DT)。结果:高血压组左室舒张功能多数表现为左室松弛功能减低,E/A<1,DT及IVRT延长;冠心病组多表现为左室松弛功能减低与顺应性减低的交界期或表现为左室顺应性减低,E/A正常,或A峰减低、DT及IVRT缩短。以上与对照组有显著性差异。结论:冠心病及高血压(尚未出现左室肥厚时)均有左室舒张功能异常,但两者表现不同,可通过彩色多普勒在早期测量左室舒张功能。

Objective To estimate the relationship of left ventricular diastolic function between Hypertension and Coronary Artery Diseases.Method Pulsed-wave-doppler echocardiography were performed in35patients with hypertension,20patients with Coronary Artery Diseases,21healthy controls for observing E/A at mitral orifice,E/Arate,deceleration time(DT),isovolumic relaxation time(IVRT).Results Compared with healthy controls,patients in Hypertension group typically exhibit a doppler pattern of 'delayed relax ation';have a significant decreased E/A rate,increased DT and IVRT.Coronary Artery Diseases group exhibit a doppler pattern of among'delayed relaxation'and'restrained relaxation'dysfunction or restrained relaxation dysfunction,have a significant decreased DT and IVRT,E/A normal or decreased.Conclusion Both patients in Hypertension and Coronary Artery Diseases have left ventric-ular diastolic dysfunction,but different from Doppler pattern.Color Doppler indicate for left ventricular diastolic dysfunction.
