伽玛刀联合外照射治疗鼻咽癌的临床研究

Gamma knife combined with the conventional external radiotherapy for NPC

目的 探讨伽玛刀在鼻咽癌治疗中的应用价值。方法 经病理确诊的鼻咽癌 94例 ,伽玛刀应用组 5 9例 ,先行鼻咽部癌肿伽玛刀治疗 ,1～ 2周后再对鼻咽部亚临床灶进行外照射 ,并根据颈淋巴结转移情况行颈部预防性或根治性放疗 ;对照组 35例 ,单纯行常规放射治疗。结果 全部病例经 1～ 3年随访 ,两组鼻咽癌的局部控制率分别为 98 3%和 94 2 % (P >0 0 5 )。而伽玛刀应用组常见放疗并发症发生率明显少于对照组 (P <0 0 5或 0 0 1 )。

Ojective To evaluate the value of gamma knife in the treatment of nasopharyngeal carcinoma(NPC). Method All 94 cases with NPC were proved by pathology, 59 cases were first treated by gamma knife, then the nasopharyngeal subclinic focuses were received conventional external radiotherapy after 1 to 2 weeks, preventive or radical radiotherapy was applied according to cervical nodal metastasis. The other 35 cases were only treated by conventional external radiotherapy as control. Results The period of follow up was 1 to 3 years, the local tumor control rates in two groups were 98 3% and 94 2% respectively(P>0 05). The incidence of radiational complications in group by gamma knife was lower than by conventional external radiotherapy(P<0 05 or 0 01). Conclusion Gamma knife combined with the conventional external radiotherapy for NPC was helpful in reducing the complication of radiotherapy.
