扁桃体周脓肿感染途径的探讨(附101例临床分析) 被引量:1

摘要 目的:探讨扁桃体周脓肿的真正发病机理和感染途径。方法:对连续诊治的101例扁周脓肿患者进行研究,其中73例患侧的扁桃体被切除,切除之扁桃体外侧面均平整。结果:通过73例组织学观察,扁桃体上极表面既无扁桃溃破,亦无脓瘘,按扁桃体上方之软腭粘膜下发现有感染及/或纤维化的小唾液腺(Weber腺),这些与扁桃体相邻的腺体在其他非扁周脓肿患者则显示正常形态与结构。结论:扁桃体周围的化脓性感染可能与Weber腺有关,故应早期切除扁桃体治疗扁周脓肿,以便充分引流。 Objective:To study the mechanism of peritonsillar abscess and its infectious route. Methods:One hundred and one cases of peritonsillar abscess were studied , and involved 73 patients were removed tonsils, the surface of the removed tonsils were smooth. Results: Neither ulceration nor pustular sinus were found on the surface of the upper part of these tonsils in the 73 cases. There were some inflammatory and/or fibrotic minor salivary glands( Weber's glands) on the top of the quinsy tonsils, the glands adjacent to the tonsils of nonquinsy patients had normal appearance. Conclusion: It suggests that the origin of peritonsillar infection may be the Weber's glands rather than extension from acutely inflamed tonsil.
