羟基喜树碱、小剂量顺铂配合氟脲嘧啶持续输注治疗晚期胃癌临床观察

Clinical Observation on Combination Chemotherapy with HCPT Low Dose DDP and 5-FU Continuous Infusion in the Treatment of Advanced Gastric Cancer

目的 观察羟基喜树碱、小剂量顺铂配合氟脲嘧啶持续输注治疗晚期胃癌的近期疗效和毒副反应。方法 3 6例晚期胃癌予以羟基喜树碱 (HCPT) 10mg静滴d1～ 5 ,顺铂 (DDP) 6mg/ (m2 ·d)静滴 ,每周连续 5天 ,氟脲嘧啶 ( 5_Fu) 0 2 5 / (m2 ·d)通过微量注射器深静脉持续输注 2 4h ,连续应用 2 1d为 1周期 ,休息 10d后重复 ,2周期以上评定疗效及毒副作用。结果 3 6例晚期胃癌中 ,CR 2例( 5 6% ) ,PR 16例 ( 4 4 4% ) ,CR +PR 5 0 0 %。毒副作用为恶心呕吐、白细胞减少和黏膜炎 ,多为Ⅰ～Ⅱ度 ,患者可以耐受。结论 羟基喜树碱及小剂量顺铂配合氟脲嘧啶持续输注治疗晚期胃癌近期疗效确切 ,毒副作用轻微 ,有临床应用价值。

Objective To evaluate the efficacy and toxicity of HCPT,low dose DDP and 5_FU continuous infusion for treatment of advanced gastric cancer.Methods A total of 36 patients were enrolled in the trial.HCPT 10 mg by intravenous titration d1～5,DDP 6mg/(m 2·d) by intravenous titration during the first day to the fifth day every week,5_Fu 0.25/(m 2·d) every day by continuous infusion for 3 weeks,every 31 days,Each patient was given at least 2 cycles.Results A complete response was observed in 2 cases and a partial response in 16 cases,and overall response rate of 50.0%.Toxic effects included nausea,leukopenia and vomiting,Main side effects were grade Ⅰ～Ⅱ.Conclusion This study confirmed that combination chemotherapy with HCPT,low dose DDP and 5_Fu continuous infusion is a safe and effective regimen for patients with advanced gastric cancer and is worthy of further clinical trial.
