摘要
目的:探讨血清中CA125、BNP及hsCRP在慢性心力衰竭(CHF)患者中的水平及临床意义。方法:90例临床确诊为慢性心力衰竭的患者,根据NYHA分级标准分为Ⅰ~Ⅱ级、Ⅲ级、Ⅳ级,同时选取30例健康体检人员作为对照组;采用电化学免疫发光法检测血清中CA125及BNP水平,用颗粒增强免疫透射比浊法检测hsCRP;比较不同组CA125、BNP、hsCRP的水平,并分析血清CA125与心功能、BNP、hsCRP的相关性。结果:与健康对照组比较,CHF组(I^II级、Ⅲ级、Ⅳ级)血清CA125、BNP及hsCRP水平逐渐升高,差异均有统计学意义(P<0.01);CA125水平与心功能级别、BNP、hsCRP呈正相关(r=0.781、0.801、0.659,P<0.01)。结论:CHF患者血清CA125水平随心功能分级升高而增高,CA125与心功能级别、BNP、hsCRP相关,三者结合可作为诊断心力衰竭的重要诊断依据。
Objective:To investigate the levels of CA125,BNP and hsCRP in patients with chronic heart failure and its clinical significance.Methods:Ninety patients diagnosed as chronic heart failure(CHF group)were selected and divided into class I-II,class Ⅲ,classⅣ according to the criterion of NYHA while 30 healthy people were used as control.Serum CA125 and BNP concentrations of patients were measured by electrochemiluminescence immunoassay.Serum hsCRP were detected by the particles reinforced immune transmission turbidimetric method.The concentrations of serum CA125,BNP and hsCRP were compared between different groups.The correlations of CA125 with cardiac function,BNP,hsCRP were analyzed.Results:Compared with control group,concentrations of serum CA125,BNP and hsCRP increased gradually in CHF group(from class I-II group,classⅢ group to classⅣgroup)and the differences were statistically significant(P <0.01).Serum CA125 concentration was positively correlated with NYHA class and concentrations of BNP and hsCRP(r=0.781,0.801,0.659,P<0.01).Conclusion:Concentration of serum CA125 in patients with CHF is increased with elevated cardiac function classification and is correlated with NYHA class,BNP and hsCRP.Combination of CA125,BNP and hsCRP can be used as the important basis in the diagnosis of heart failure.
