摘要
目的:探讨超声弹性评分与剪切波速度诊断乳腺肿瘤的临床价值及其相关性。方法:经手术病理证实的乳腺肿瘤患者243例,术前行超声弹性及声触诊组织量化技术检查,计算弹性评分及剪切波速度诊断乳腺肿瘤的特异性、灵敏性,并分析二者相关性。结果:1乳腺恶性肿瘤弹性评分及剪切波速度明显高于良性(P<0.05)。以4分为诊断界值,弹性评分诊断乳腺肿瘤良恶性的特异性为86%,灵敏性为87%;以3.91m/s为诊断界值,剪切波速度诊断乳腺肿瘤良恶性的特异性为88%,灵敏性为94%。弹性评分联合剪切波速度诊断乳腺恶性肿瘤的特异性为98%,灵敏性为90%。2弹性评分与剪切波速度有良好的相关性,r=0.687(P<0.05)。结论:弹性评分与剪切波速度诊断乳腺肿瘤具有较好的准确性及相关性,可作为诊断乳腺肿瘤互为补充的重要方法。
Objective:To study the correlation between ultrasonic elastic score and shear wave velocity in the diagnosis of breast tumor.Methods:A total of 243 cases of breast tumor confirmed by pathologic results were enrolled in this study.Ultrasound elasticity was used to obtain the elastic score of breast tumor,and the virtual touch tissue quantification technique was performed to measure the shear wave velocity in tumor center,the diagnostic specificities and sensitivities of benign and malignant tumors by the shear wave velocity and elastic score were compared and analyzed.Results:1 The elastic scores and shear wave velocities in malignant tumors were higher than those of benign tumors(P<0.05).The elastic score 4was used as the critical value to diagnose benign and tumors,the specificity and sensitivity was 86% and 87%,respectively.The shear wave velocity 3.91m/s was used as the critical value,the specificity and sensitivity was 88% and 94%,respectively.The combination of elastic score and shear wave velocity for diagnosis of the benign and malignant breast tumors,the specificity and sensitivity was 98% and 90%,respectively.2There presented a prominent correlation between elastic score and shear wave velocity(r=0.687,P<0.05).Conclusion:There is a good accuracy and correlation between elastic score and shear wave velocity in the differential diagnosis of benign and malignant breast tumors,and elastic score and shear wave velocity can be used as an alternative in the diagnosis of breast tumor.
