大学生夜磨牙患者垫治疗前后肌电变化特征的研究 被引量:2

摘要 目的:研究大学生夜磨牙症患者在使用弹性垫治疗前后,应用Bio-Pak肌电图仪测量咀嚼肌的肌电参数变化和临床治疗效果。方法:使用弹性垫对大学生夜磨牙症患者进行治疗,并应用Bio-Pak肌电图仪在治疗前和治疗后1、3、6个月测量患者咀嚼肌的肌电参数变化,观察临床疗效。结果:1下颌姿势位检测结果:治疗前,实验组两侧颞肌前束和咬肌均值电位明显高于对照组(P<0.05),治疗后均逐渐降低,6个月后,两侧颞肌前束均值电位和对照组比较无统计学差异(P>0.05);治疗前下颌姿势位两侧颞肌前束和咬肌肌电对称率以及咀嚼肌活动总体不对称性指数与对照组有差异(P<0.05),治疗6个月后基本与对照组无明显差异(P>0.05)。2牙尖交错位最大力紧咬时检测结果:治疗前,实验组两侧颞肌前束和咬肌平均峰值电位明显低于对照组(P<0.05),治疗后均逐渐增高,治疗后6个月,两侧颞肌前束和咬肌平均峰值电位和对照组比较无统计学差异(P>0.05);实验组治疗前两侧咬肌肌电对称率以及咀嚼肌活动总体不对称性指数与对照组有差异(P<0.05),治疗6个月后基本与对照组无明显差异(P>0.05);两侧颞肌前束肌电对称率治疗前后与对照组均无差异(P>0.05)。结论:使用弹性垫治疗磨牙症,可以有效纠正磨牙症患者的肌功能紊乱,并使咀嚼肌功能和形态均趋于正常。 Objective:To study the treatment effect of elactic Occlusal splint on bruxism patients by comparing the masticatory muscle electromyography parameters tested with Bio-Pak EMG instrument before and after treatment. Methods:Elactic Occlusal splint were used in the treatment of bruxism patients among college students and the patients' masticatory muscle electromyography were tested with Bio-Pak EMG instrument were during,1 month after,3 months after and 6 months after the treatment. Then the clinical effect were observed. Results:1Detection results of mandibular postural position(MPP):before treatment,the experimental group has significantly higher mean value of postural position on both sides of the anterior temporalis(TA) and masseter muscle(MM) than the control group(P < 0. 05). After treatment,the difference were reduced until,when six months after treatment,there no longer exist any significant difference between the two groups(P > 0. 05). Before treatment,there existed significant difference between the two groups in Astot and EMG symmetric rate before lateral TA and MM treatment(P <0. 05),while about 6 months after treatment,the difference disappears(P > 0. 05). 2The maximum bite force detection results at intercuspal position(ICP):there were significant differences between experimental group and the control group on both sides of the TA and MM average peak potential before treatment(P < 0. 05). After 6 months of treatment,the experimental group on both sides of the TA and MM average peak potential increased,and showed no significant difference with the control group(P > 0. 05). The symmetric rate and Astot of TA and MM EMG of the experimental group are significantly higher than the control group(P < 0. 05),while after 6 months of treatment,this difference decreased to be insignificant(P > 0. 05). There were no significant difference between the two group in TA EMG symmetric rate both sides before and after the treatment(P > 0. 05). Conclusion:Elastic occlusal splint can effectively correct muscle dysfunction and improve the functions and structure of the masticatory muscles until they are normal.
