2014—2018年云南省孕产妇HIV梅毒和HBV检测结果

Status of HIV,syphilis and HBV detection among pregnant women in Yunnan during 2014-2018

目的分析2014—2018年云南省孕产妇人群艾滋病病毒(HIV)、梅毒和乙型肝炎(简称乙肝)病毒(HBV)血清学检测情况、检测阳性率和地区分布,为云南省消除母婴传播提供参考。方法利用云南省预防艾滋病、梅毒和乙肝母婴传播管理信息监测中的相关数据,收集云南省孕产妇妊娠不同时期HIV、梅毒和HBV血清学检测率和检测阳性率等指标,采用χ~2检验、趋势χ~2检验分析孕产妇HIV、梅毒和HBV检测率年度变化,以及孕产妇人群检测阳性率的变化及分布。结果 2014—2018年间,云南省共报告产妇3 115 046人,HIV、梅毒和HBV的累计检测率分别为99.88%(3 111 427人)、99.85%(3 110 492人)、99.85%(3 110 517人)。孕产妇孕期和仅产时HIV检测阳性率分别为0.25%(9 561/3 793 548)和0.34%(539/158 114),梅毒孕期和仅产时检出阳性率分别为0.30%(11 241/3 794 241)和0.56%(887/158 157),HBV孕期和仅产时检出阳性率分别为2.83%(107 301/3 792088)和6.34%(10 031/158 157)。2018年云南省孕产妇中HIV、梅毒和HBV检出阳性比例的地区分布差异有统计学意义。结论云南省2014—2018年间的孕产妇HIV、梅毒和HBV检测率均保持较高的水平,仅产时检测发现感染孕产妇的比例高于孕期,提示在提高检测覆盖率的同时,应进一步提高孕早期的检测比例,重点关注仅产时检测阳性率和阳性数较高的地区,减少母婴传播病例的发生。

Objective To analyze the results of serological testing of HIV,syphilis and HBV among pregnant women in Yunnan province from 2014 to 2018,and to provide reference for elimination of mother to child transmission.Methods Data of HIV,syphilis and HBV,positive rates and related indicators of pregnancy were collected.Chi-square tests were used to analyze the annual changes of HIV,syphilis and HBV infection.Results The detection rates of HIV,syphilis and HBV were 99.88%(3 111 427),99.85%(3 110 492)and 99.85%(3 110 517).The detection rate in early pregnancy increased and that at delivery decreased year by year.The positive rates of HIV,syphilis and HBV detection at delivery were higher than those in pregnancy.The regional distribution differences of HIV,syphilis and HBV positive rates among pregnant women were statistically significant in 2018.Conclusion The detection rates of HIV,syphilis and HBV are high.However,the positive rate at delivery is higher than that in pregnancy.We must pay more attention to the areas where there are high positive rates and positive numbers at delivery to reduce the incidence of mother-to-child transmission.
