孕期补钙与产时补钙防治宫缩乏力性产后出血的护理观察

To observe the clinical nursing of pregnancy and intrapartum calcium supplement to prevent and control the contractions fatigue sex postpartum hemorrhage

目的:观察孕期补钙与产时补钙防治宫缩乏力性产后出血的临床护理。方法:收治经阴道分娩产妇106例,随机分为两组,孕期补钙组及产时补钙组,孕期补钙组16周开始每日口服钙剂至分娩,对照组孕期未口服钙剂,仅在进入第二产程后予5%葡萄糖100ml加10%葡萄糖酸钙15ml静滴。结果:孕36周及产前血钙水平比较,孕期补钙组高于产时补钙组,差异有统计学意义(P<0.05)。与产时补钙组相比,孕期补钙组开始有效子宫收缩时间较短,子宫出血量较少,差异有统计学意义。产时补钙组观察到2例>15分钟子宫不收缩和1例产后24小时反复阴道大量出血的病例,孕期补钙组未观察到上述情况。结论:在孕期按推荐剂量补钙并监测血钙水平对防治产后出血的效果优于产时补钙,在临床护理中对规律产检的孕产妇进行补钙情况的跟踪和监督对预防产后出血有重要意义。

Objective:To observe the clinical nursing of pregnancy and intrapartum calcium supplement to prevent and control the contractions fatigue sex postpartum hemorrhage.Methods:To admit 160 cases of vaginal delivery puerpera from May 2008 to May 2010.There were randomly divided into two groups,pregnancy calcium supplement group and intrapartum calcium supplement group.From 16 weeks,the pregnancy calcium supplement group began to daily oral calcium until delivery,the control group pregnancy have not oral calcium,only in the second stage of labor,were gived intravenous infusion with 5% glucose 100ml plus l0% calcium gluconateof 15ml.Results:Comparison the serum calcium level in 36 weeks of pregnancy and prenatal,the pregnancy calcium supplement group was higher than the intrapartum calcium supplement group,with significant difference(P< 0.05).Compared with the intrapartum calcium supplement group,the pregnancy calcium supplement group began to effective uterine contraction in a shorter time,less bleeding,there was significant difference.In the intrapartum calcium supplement group, observed 2 cases of more than 15 minutes of no shrinkage and 1 cases of postpartum 24 hours repeated vaginal bleeding,the pregnancy calcium supplement group were not observed in the above.Conclusion:During pregnancy,according to the recommended dose to supple calcium and monitor the blood calcium level,have a better effect of prevention and treatment of postpartum hemorrhage than intrapartum calcium supplement.In clinical nursing,tracking and monitoring calcium situation is important for the prevention of postpartum hemorrhage on regular prenatal maternal.
