单纯修补、胃部分切除术治疗急性胃穿孔的临床效果对比

Comparison of the clinical effect of simple repair,partial gastrectomy in treatment of acute gastric perforation

目的:分析和对比单纯修补、胃部分切除术治疗急性胃穿孔的临床效果。方法:收治急性胃溃疡穿孔患者62例,作为研究对象,采取数字标记法随机将上述选取对象分成A组和B组,A组采取单纯修补术进行治疗,B组采取胃部分切除术进行治疗,观察和记录两组在手术时间、住院时间、术后并发症发生率及溃疡复发率等指标,并进行分析和对比。结果:A组32例患者手术时间和住院时间分别(42.92±5.77)分钟和(7.04±1.86)天,术后并发症发生率3.13%;B组30例患者手术时间和住院时间分别(165.28±17.72)分钟和(11.34±3.46)天,术后并发症发生率33.33%,两组间上述3项指标对比差异有统计学意义(P<0.05);但两组在术后溃疡症状复发情况对比上差异无统计学意义(P>0.05)。结论:对于急性胃穿孔患者,选取单纯修补术进行治疗,对于缩短手术时间,降低术后并发症较胃大部切除术有着明显优势,同时还有助于缩短患者的康复进程,这对于改善患者预后,提升生活质量有着明显的积极意义。

Objective:Analysis and comparison the clinical effect of simple repair,partial gastrectomy in treatment of acute gastric perforation.Methods:To collect 62 cases of patients with acute gastric perforation from December 2011 to August 2013,as the research object,adopt digital labeling the selected objects randomly divided into A group and B group,A group patients were treated by simple closure operation,B group took partial gastrectomy for treatment,observe and record operation time, hospitalization time,postoperative complications and recurrence rate of ulcer index in the two groups of patients,analysis and comparison.Results:The operation time and hospitalization time of 32 patients in A group were(42.92±5.77) minutes and(7.04± 1.86) days,the incidence of postoperative complications was 3.13%.The operation time and hospitalization time of 30 patients in B group were(165.28±17.72) minutes and(11.34±3.46) days,the incidence of postoperative complications was 33.33%,there were significant differences and statistical significance of two groups between these three indicators contrast(P<0.05);but the postoperative recurrence of ulcer symptoms of the two groups had no significant difference(P>0.05).Conclusion:For patients with acute gastric perforation,compare with partial gastrectomy,simple closure operation have obvious advantages with shorten the operation time,reducing the postoperative complications,also help to shorten the rehabilitation process of patients,to improve the prognosis of the patients,there is positive significance to improve quality of life,this technique is worth popularizing in clinic.
