摘要
上海市长宁区在全科服务团队模式的基础上实施以居委为服务单位的家庭医生制度工作,本文在长宁某社区实际工作的基础上探讨此模式下家庭医生的工作职责,从居民健康信息管理的职责、健康生活行为干预指导的职责、与居民家庭建立伙伴关系的职责、健康服务路径指引的职责4个方面进行了阐述。
We implemented the family doctor system with JuWei for service units based on the general service team mode in Changning District of Shanghai City.This paper discussed the work responsibilities of family doctors in this mode based on the actual work in community.We discussed it in four aspects,including the health information management duty,health behavior intervention guidance duty,a partnership with household duty and responsibilities of health service path.
