老年患者呼吸机相关性肺炎的危险因素分析及防治措施

Analysis of the risk factors and preventive measures of ventilator associated pneumonia in elderly patients

目的:对老年患者发生呼吸机相关性肺炎(VAP)的危险因素及防治措施进行分析和总结,以期降低老年患者呼吸机相关性肺炎的患病率。方法:回顾性研究ICU科接受呼吸机治疗的老年患者,对比分析VAP患者和非VAP患者的临床资料,对VAP的危险因素和防治措施进行分析和总结。结果:两组的APACHEⅡ评分、血浆白蛋白水平、机械通气时间、抗生素连续使用时间、昏迷持续时间等方面比较,差异均有统计学意义(P<0.05)。结论:老年患者发生呼吸机相关性肺炎的危险因素较多,根据发生VAP的危险因素制定并实施有针对性的防治措施,对降低老年患者VAP的发生率有着重要作用。

Objective:To analyze and summary the risk factors and preventive measures of ventilator associated pneumonia in elderly patients,to reduce the prevalence of ventilator associated pneumonia in elderly patients.Methods:The elderly patients with ventilator therapy in ICU were analyzed retrospectively.We compared and analyzed the clinical data of patients with VAP and non VAP patients,the risk factors and preventive measures about VAP were analyzed and summarized.Results:The APACHE domain score,plasma albumin level,mechanical ventilation time,continuous use time of antibiotics,coma duration and other aspects of two groups of patients were compared,the differences were statistically significant(P<0.05).Conclusion:The risk factors of ventilator associated pneumonia in elderly patients are more.According to the risk factors of VAP to develop and implement targeted prevention and control measures has important effect to reduce the incidence of VAP in elderly patients.
