摘要
目的:分析磁共振弥散成像对急性脑梗死的诊断价值。方法:收治急性脑梗死患者60例,对所有患者实施磁共振弥散成像扫描和液体衰减反转恢复检查,对两种检查结果进行对比分析。结果:本组60例患者中,16例超急性脑梗死,31例急性脑梗死,13例亚急性脑梗死,所有患者通过磁共振弥散成像均显示为高信号。在超急性脑梗死期,磁共振弥散成像可将常规的T2WI像无法诊断的早期脑梗死灶显示出来。本组16例超急性梗死灶通过磁共振弥散成像全部显示出来,检出率100.0%,明显高于T2WI;液体衰减反转恢复检查主要对脑室旁区的微小病变、大脑半球表面、灰白质交界、基底池周围、脱髓鞘或者脑白质变性的显示具有更高的敏感性。结论:磁共振弥散成像对急性脑梗死具有良好的诊断价值,能及早地将病灶清晰地显示出来,从而提高急性脑梗死的早期诊断率。同时,磁共振弥散成像对急性脑梗死的诊断率明显优于常规MR,在应用磁共振弥散成像诊断急性脑梗死过程中应尽可能地结合液体衰减反转恢复检查及T2WI以提高新旧梗死灶的鉴别率。
Objective:To explore the diagnostic value of magnetic resonance diffusion imaging for acute cerebral infarction.Methods:60 patients with acute cerebral infarction were selected.All patients were examined by magnetic resonance imaging scans and fluid attenuated inversion recovery.Two kinds of test results were compared.Results:In 60 patients,16 cases were super acute cerebral infarction,31 cases were acute cerebral infarction,13 cases were sub-acute cerebral infarction,all patients showed high signal in magnetic resonance imaging.In the super acute cerebral infarction,magnetic resonance diffusion imaging can display cerebral infarction early which can not display by conventional T2WI image.16 cases of super acute cerebral infarction showed up in diffusion magnetic resonance imaging,detection rate was 100%,the detection rate was significantly higher than that of T2WI;fluid attenuated inversion recovery test was more sensitive to the paraventricular region of small lesions,the brain surface,gray white matter junction,basal cistern around,demyelination or cerebral white matter degeneration.Conclusion:Magnetic resonance diffusion imaging has good diagnostic value in acute cerebral infarction,it can display lesions as soon as possible,so as to improve the rate of early diagnosis of acute cerebral infarction.At the same time,magnetic resonance imaging in diagnosis of acute cerebral infarction were significantly better than conventional MR.In the application of diffusion-weighted imaging in diagnosis of acute cerebral infarction process,we should be as much as possible with fluid attenuated inversion recovery test and T2WI in order to improve the identification rate of infarct of old and new.
