开展社区高血压病患者健康管理工作的成效分析

Effectiveness analysis of development of community health management in patients with hypertension

目的:总结高血压病患者健康管理工作的成效。方法:分析2012年12月-2013年12月上城辖区开展高血压病患者健康管理工作1年间的患者管理资料,总结本社区高血压病患者健康管理率、规范健康管理率、血压控制率以及管理前后患者血压变化。结果:高血压病患者健康管理率19%(省考核要求20%);规范健康管理率85%(省考核要求30%);管理人群血压控制率80%;管理后患者血压水平均明显低于管理前,差异有统计学意义(P<0.05)。结论:开展社区高血压病患者健康管理工作有利于对并发症及其预后进行早期干预。

Objective:To summary the effect of health management in patients with hypertension.Methods:Through developing community health management in patients with hypertension in Shangcheng District from December 2012 to December 2013,to analyze patients' management information in this year,and summary health management rate,the normative health management rate,blood pressure control rate and the blood pressure changes of patients before and after management of patients with hypertension in this community.Results:Health management rate of patients with hypertension was 19%(provincial assessment requirement was 20%).The normative health management rate was 85%(provincial assessment requirement was 30%).Blood pressure control rate 80%.The blood pressure level of patients after management were lower than that of before management.It had significant difference(P<0.05).Conclusion:The development of community health management in patients with hypertension is availlable to have early intervention of its complications and prognosis.
