摘要
目的:探讨64层螺旋CT肺动脉造影在不同肺动脉分支水平肺动脉内栓子的显示率及其诊断价值。方法:将收治的40例肺栓塞患者的64层螺旋CT肺动脉造影图像分别重建为0.75 mm、1.5 mm、3.0 mm和5.0 mm层厚,记录显示清楚的各分支水平的肺动脉内有无内栓子,比较四组图像对肺动脉分支和动脉内栓子的显示情况。结果:0.75mm和1.5 mm层厚在各水平肺动脉显示率差异无统计学意义(P>0.05),1.5 mm厚层优于5.0 mm和3.0 mm(P<0.05)。血栓显示方面,0.75 mm和1.5 mm厚层在亚段以上肺动脉栓子的显示上差异无统计学意义(P>0.05),3.0 mm与5.0mm层厚显示叶、段、亚段肺动脉差异有统计学意义(P<0.05),1.5 mm层厚的亚段肺动脉栓塞血管显示率显著优于3.0 mm(P<0.05)。结论:重建1.5 mm层厚图像在显示周围肺动脉和亚段以上肺动脉内栓子方面具有良好的诊断作用,具有重要的诊断和应用价值,值得在临床上推广使用。
Objective:To investigate the display rate of 64 slice spiral CT pulmonary angiography on emboli in different branches of the pulmonary artery in pulmonary artery and its diagnostic value.Methods:40 cases with pulmonary embolism were treated with64 slice spiral CT pulmonary angiography images,reconstructed for 0.75 mm,1.5 mm,3.0 mm and 5.0 mm layer thickness,and recorded that if the thrombus exist in the pulmonary artery of each branch level which can show clearly,compared display of four groups image on the branches and arterial embolus of the pulmonary artery.Results:Each level of the display rate of pulmonary artery in 0.75 mm and 1.5 mm layer thickness have no significantly difference(P0.05).1.5 mm thick layer is better than 5.0 mm and 3.0 mm(P0.05).In thrombosis display,subsegmental pulmonary embolus of the above in thick layer of 0.75 mm and 1.5 mm showed no significant difference(P0.05).Leaf,section,sub segmental pulmonary arteries in 3.0 mm and 5.0 mm layer thickness showed significant difference(P0.05).The display rate of subsegmental pulmonary embolism vascular thickness of 1.5 mm was better than that of 3.0 mm(P0.05).Conclusion:Reconstruction slice thickness of 1.5 mm image has good effect in diagnosis of peripheral pulmonary arteries and sub segment display above the emboli.It has important value in diagnosis and application.So it is worthy of clinical promotion.
