基层医院开展白内障超声乳化术体会

The Experience of developing cataract phacoemulsification in primary hospital

目的:探讨基层医院从白内障囊外摘除术转型到白内障超声乳化人工晶体植入术。方法:采用普通手术设备,对12个月内完成207眼白内障超声乳化摘除并人工晶体植入手术的临床资料进行分析。结果:经手术后3个月随访,术后视力≥0.3者占87.4%,其中视力≥0.8者占72.9%,其中后囊破裂6眼,悬韧带离断3眼,囊袋内植入198眼,睫状沟植入6眼,双襻悬吊1眼,单襻悬吊1眼,超声时间平均0.5 min,超声能量33%~66%。结论:基层医院虽设备较差,初学者并不能完全撑握超声乳化术,但只要充分准备,细心操作,完全能够开展白内障超声乳化人工晶体植入术。

Objective:To investigate the transformation from extracapsular cataract extraction to phacoemulsification and intraocular lens implantation in primary hospital.Methods:Using the normal operation equipment,207 eyes with cataract phacoemulsification and intraocular lens implantation operation within 12 months were selected.Their clinical data were analyzed.Results:Patients were followed-up for 3 months after the operation.The postoperative visual acuity≥0.3 accounted for 87.4%.The visual acuity≥0.8 accounted for 72.9%.6 eyes with posterior capsule rupture;3 eyes with zonule rupture;198 eyes implanted in the capsular bag;6 eyes with IOL implantation in ciliary sulcus;1 eye of double loop suspension;1 eye of single loop suspension.The ultrasonic time averaged 0.5 minutes.The ultrasonic energy was 33% ~66%.Conclusion:Although the equipment of the primary hospital is poor,and beginners cannot fully grasp the phacoemulsification,but so long as adequate preparation and careful operation,it is fully capable to develop cataract phacoemulsification and intraocular lens implantation.
