社区管理对高血压患者依从性和控制率的影响

Influence of community management on the compliance and control rate of hypertension patients

目的:探讨社区管理对高血压患者依从性和控制率的影响。方法:2012年1月-2013年2月收治原发性高血压患者70例,收治同期高血压患者20例作为对照组。对照组不进行特殊管理,单靠患者自愿咨询等;管理组进行社区管理,建立健康档案,定期进行随访和健康教育。结果:管理组经过管理后物质生活状态、社会领域、心理领域、躯体功能明显改善,优于对照组,两组比较差异有统计学意义(P<0.05)。两组治疗前后脂蛋白、甘油三酯、总胆固醇、舒张压、收缩压均有明显改善,治疗前后比较差异有统计学意义(P<0.05);治疗后,管理组脂蛋白、甘油三酯、总胆固醇、舒张压、收缩压明显优于对照组,两者比较差异有统计学意义(P<0.05);管理组严格按照医嘱依从的人员67例,依从率95.7%,对照组依从的人员10例,依从率50.0%,管理组血压恢复正常51例,控制率72.9%,对照组血压恢复正常8例,控制率20.0%,两组比较差异有统计学意义(P<0.05)。结论:社区管理可以有效提高高血压患者依从性和控制率,是社区控制高血压的关键。

Objective:To explore the influence of community management on the compliance and control rate of hypertension patients.Methods:70 patients with primary hypertension were selected from January 2012 to February 2013.20 patients with hypertension were selected at the same time as the control group.The control group were without special management,rely on patients with voluntary counselling.The management group were given community management,first established health records,regularly given follow-up and health education.Results:The material life condition,social field,psychological domain and physical function of the management group were improved after management,which were better than those of the control group,and there were significant difference between two groups(P<0.05).The lipoprotein,triglycerides,total cholesterol,diastolic blood pressure and systolic blood pressure of two groups were all significantly improved before and after treatment,and there was significant difference before and after treatment(P<0.05).The lipoprotein,triglycerides,total cholesterol,diastolic blood pressure and systolic blood pressure of the management group after treatment were significantly better than those of the control group after treatment,and the difference was statistically significant(P<0.05).The personnel in strict accordance with the prescribed compliance of the management group were 67 cases,and the compliance rate was 95.7%.The compliance personnel of the control group were 10 cases,and the compliance rate was 50.0%.The blood pressure of 51 cases in the management group returned to normal,and the control rate was 72.9%.The blood pressure of 8 cases in the management group returned to normal,and the control rate was 20.0%.The differences of two groups were statistically significant(P<0.05).Conclusion:Community management can effectively improve the compliance and control rate of hypertension patients.It is the key to community control of hypertension.
