摘要
目的:探讨西替利嗪滴剂联合雾化布地奈德混悬液治疗儿童支气管哮喘合并变应性鼻炎的临床疗效。方法:2012年3月-2014年4月收治支气管哮喘合并变应性鼻炎患儿58例,随机分为两组,各29例,两组在相同对症支持治疗的基础上,治疗组给予口服西替利嗪滴剂,3~6岁2.5 mg/次,2次/d,>6岁5 mg/次,2次/d,雾化吸入布地奈德混悬液2 m L,2次/d;对照组同法给予雾化吸入布地奈德混悬液,同时口服氯雷他定5~10 mg,1次/d,每晚睡前服,疗程均为2个月。观察两组患儿治疗后的鼻塞、喷嚏、流涕、咳嗽、喘息等改善情况。结果:治疗组鼻炎治疗总有效率89.7%,对照组总有效率58.6%。治疗组鼻塞、喷嚏、流涕等鼻炎症状改善优于对照组,两组比较差异具有统计学意义(P<0.05);治疗组哮喘治疗总有效率93.1%,对照组总有效率65.5%,治疗组咳嗽、喘息症状改善优于对照组,两组比较差异具有统计学意义(P<0.05)。结论:西替利嗪滴剂联合雾化布地奈德治疗儿童支气管哮喘合并变应性鼻炎具有良好的临床疗效与安全性,依从性好,不良反应少。
Objective:To investigate the clinical efficacy of cetirizine drops combined with nebulized budesonide suspension in the treatment of children with bronchial asthma complicated with allergic rhinitis.Methods:58 children with bronchial asthma complicated with allergic rhinitis were selected from 2012 March to April 2014.They were randomly divided into two groups with29 cases in each.Patients in both of the groups were given the same basic treatment.Moreover,the treatment group were given oral cetirizine drops,2.5 mg every time for 3~6 years old children,2 times/d;for children who was more than 6 years old,they were given 5 mg every time,2 times/d,inhalation of budesonide suspension in 2 m L,2 times/d.The control group were given budesonide atomization inhalation with the same methods above,and also given 5~10 mg oral loratadine at the same time,1 time/d at every night before bedtime,the course of treatment both were 2 months.Then we observed the improvement in sneezing,runny nose,stuffy nose,cough,wheezing of two groups after treatment.Results:The total effectiveness of rhinitis in the treatment group was 89.7%,and that in the control group was 58.6%.The improvement of nasal congestion,sneezing,runny nose and other symptoms of rhinitis in the treatment group was better than that in the control group,and the differences were statistically significant between the two groups(P<0.05).The total effectiveness of asthma in the treatment group was 93.1%,and that in the control group was 65.5%.The improvement in cough and wheezing symptoms of the treatment group were better than the control group,and the differences were statistically significant between the two groups(P<0.05).Conclusion:Cetirizine drops combined with nebulized budesonide in the treatment of children with bronchial asthma complicated with allergic rhinitis has good clinical efficacy.It is safe and has good compliance.It also has less adverse reaction.
