无创呼吸机治疗COPD呼吸衰竭及慢性心功能不全的临床疗效观察

摘要 目的:探讨无创呼吸机治疗COPD呼吸衰竭及慢性心功能不全的临床疗效。方法:2011年1月-2013年10月收治COPD、呼吸衰竭合并有慢性心功能不全患者68例,随机分为治疗组和对照组,各34例,对照组给予常规一般治疗,治疗组在对照组的基础上配合无创呼吸机治疗,治疗48 h后比较两组患者的心功能、心率、血气分析、左室射血分数等。结果:治疗后,治疗组Pa O2、Sp O2、p H值、PO2/Fi O2、LVEF明显升高,Pa CO2、HR、RR明显下降,与对照组相比,差异有统计学意义(P<0.05)。结论:Bi PAP呼吸机治疗慢性心力衰竭患者,可以改善患者心功能及临床症状,值得临床推广应用。 Objective:To investigate the clinical efficacy of noninvasive ventilation in the treatment of COPD with respiratory failure and chronic heart failure.Methods:68 patients with COPD,respiratory failure with chronic heart failure were selected from January 2011 to October 2013.They were randomly divided into the treatment group and the control group with 34 cases in each.The control group were given the conventional treatment.The treatment group were given noninvasive ventilator therapy based on the control group.After 48 h treatment,we compared the heart function,heart rate,blood gas analysis,left ventricular ejection fraction between two groups.Results:After the treatment,the Pa O2,Sp O2,p H,PO2/Fi O2,LVEF of the patients in the treatment group were increased significantly;the Pa CO2,HR,RR were decreased significantly.Compared with the control group,the difference was statistically significant(P<0.05).Conclusion:Bi PAP ventilation in the treatment of patients with chronic heart failure can improve cardiac function and clinical symptoms,so it is worthy of clinical application.
