儿童病毒性脑炎的MRI诊断及FLAIR序列的临床应用 被引量:2

摘要 目的:分析儿童病毒性脑炎的MRI影像学表现的早期诊断。方法:2012年6月-2014年7月收治病毒性脑炎患儿32例,收集MRI影像检查资料,结合临床症状及病史对其MRI影像学表现进行分析。结果:MRI检查病灶主要位于双侧丘脑、基底节区、中脑、大脑半球皮层下区,大部分丘脑及基底节区病灶呈较对称性分布。T1WI像上病灶呈稍低信号21例,病灶表现不明显11例;T2WI像上呈稍高、高信号32例;FLAIR像上均发现高、稍高信号病灶。结论:儿童病毒性脑炎发病以夏秋季节多,病灶主要分布于丘脑、基底节区等核团区,大脑半球主要累及皮层下区,常规SE T1WI不敏感,早期病灶T2WI呈稍高信号易漏诊,FLAIR序列能够敏感地、清楚地显示出早期病灶。 Objective:To analyze the early diagnosis of MRI imaging manifestations of children viral encephalitis.Methods:32children with viral encephalitis were selected from June 2012 to July 2014.The MRI imaging examination data were selected.The MRI imaging manifestations were analyzed combined with clinical symptoms and medical history.Results:MRI examination:the lesions mainly located in bilateral thalamus,basal ganglia region,midbrain,cerebral hemisphere subcortical area,most of thalamus and basal ganglia region lesions showed a symmetrical distribution.In T1 WI images,the lesions showed slightly low signal in 21cases;the lesions showed not obvious in 11 cases.In T2 WI images,the lesions showed slightly higher;high signal in 32 case.The higher,slightly high signal lesions were found in FLAIR images.Conclusion:The incidence of children viral encephalitis is majorly in the summer season.The lesions are mainly distributed in the thalamus,basal ganglia region and other nucleus area.The cerebral hemisphere is mainly involving subcortical area.The conventional SE T1 WI is not sensitive.The early T2 WI lesions with slightly higher signal are easy to be missed.The sequence of FLAIR can sensitively,clearly showing the early lesions.
