糖尿病社区综合干预管理模式分析

Analysis of diabetes community comprehensive intervention management model

目的:为加强人们对糖尿病的重视程度、强化对糖尿病的管理控制、提高糖尿病患者生存质量,社区综合干预管理成为有效管理模式。本文总结了社区综合干预管理模式对糖尿病患者病情控制的有效防治经验,得出规范化的社区综合干预管理对糖尿病的预防与控制有极大的助益,期望更多的社区强化推广管理工作。

In order to strengthen the attention degree of diabetes,increase the reinforcement of diabetes management control,improve the life quality of diabetes patients and community comprehensive intervention management becomes the effective management model.This paper summarizes effective prevention and control experience of community comprehensive intervention management model on disease control of diabetes patients.Normalized community comprehensive intervention management model has great help on the prevention and control of diabetes.We expect more communities to strengthen the promotion of management.
