小剂量轻比重腰麻-硬膜外联合麻醉对老年下肢骨折的有效性

The effect of small doses of hypobaric spinal-epidural anesthesia in elderly lower extremity fractures

目的:讨论研究小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折治疗中的有效性。方法:2011年2月-2013年12月收治下肢骨折老年患者78例,随机分为试验组和对照组,每组39例。试验组采取小剂量轻比重腰麻-硬膜外联合麻醉,对照组采取连续硬膜外麻醉。并且将两组患者麻醉过程中的实际效果进行记录分析,通过对两组施用的麻醉药物剂量、麻醉后感觉阻滞的起效时间以及麻醉的完善总用时的对比,研究小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折中的临床应用效果。结果:通过对两组患者麻醉状况的对比分析可知,试验组中对患者用的药物剂量比对照组要少,且差异具有统计学意义(P<0.05);试验组中麻醉后感觉阻滞起效的时间比对照组要少,且差异具有统计学意义(P<0.05);并且试验组麻醉完善用时比对照组也较少,且差异具有统计学意义(P<0.05)。结论:在老年患者下肢骨折手术中采用小剂量轻比重腰麻-硬膜外联合麻醉法较为安全有效,并且还有药物用量少、阻滞完善以及镇痛效率高等特点,适合于临床广泛推广应用。

Objective:To discuss the effect of small doses of hypobaric spinal-epidural anesthesia in elderly lower extremity fractures.Methods:78 elderly patients with Lower extremity fractures were selected from February 2011 to December 2013.They were randomly divided into the experimental group and the control group,39 cases in each.Patients in the experimental group were used a small dose of hypobaric spinal-epidural anesthesia,and the control group of patients were used continuous epidural anesthesia.The two groups of patients during anesthesia,recording the actual effect and analyzing,through the improvement of the two groups of anesthesia drug dose,the onset time of anesthesia feeling after anesthesia and anesthesia total time contrast,to research the effect of small doses of hypobaric spinal-epidural anesthesia in elderly lower extremity fractures.Results:Through the comparison of the two groups of patients with anesthesia conditionanalysis,the experimental group were treated with a dose of the drug was relatively less than the control group,there was statistically significant(P<0.05);in the experimental group after anesthesia,the onset time of sensory block of time was less than the control group,there was statistically significant(P<0.05);the anesthesia perfect time of the the experimental group was less than the control group,and there was statistically significant(P<0.05).Conclusion:The effect of small doses of hypobaric spinal-epidural anesthesia in elderly lower extremity fractures is safe,the dosage of drugs is small,complete block and analgesia with high efficiency.
