骨盆骨折并发失血性休克的急诊治疗分析

Emergency treatment analysis of pelvic fractures combined with hemorrhagic shock

目的:探究骨盆骨折并发失血性休克的急诊治疗方案。方法:2014年1-11月收治骨盆骨折并发生失血性休克患者24例,接受输液输血治疗后,采用相应Seldinger完善技术实施造影明确出血灶,利用明胶海绵来实行动脉栓塞。结果:3例患者在发生弥漫性血管内凝血后死亡(12.5%)。15例患者经过紧急治疗后有所好转(62.5%)。6例患者经过紧急治疗后,出现部分肌肉以及皮肤感染坏死,其中3例患者(25%)的盆腔脏器发生了并发症,但通过治疗后,逐渐好转。结论:骨盆骨折并发失血性休克患者给予输血输液、止血固定治疗后能够提升骨盆骨折的生存质量,减少死亡率,值得推广应用。

Objective:To explore the emergency treatment of pelvic fracture complicated with hemorrhagic shock.Methods:24 cases of patients with pelvic fractures and bleeding shock from January to November in 2014 were given blood transfusion treatment and the hemorrhage nudi were confirmed by implementation of imaging using Seldinger perfect technical,and the artery embolization were practiced with gelatin sponge.Results:3 cases of patients (12.5%)died after diffuse intravascular coagulation.15 cases of patients (62.5%) were improved after emergency treatment.6 cases of patients had part of the muscle and skin necrosis infection after emergency treatment,including 3 cases of patients (25%) had pelvic viscera complications,but gradually improved after treatment.Conclusion:Blood transfusion and hemostatic fixed treatment could improve the survival quality of patients with pelvic fracture complicated with hemorrhagic shock and reduced the mortality rate,worthy of popularization and application.
