mf-ERG联合OCT对曲安奈德玻璃体腔注射治疗黄斑囊样水肿的疗效评价

摘要 目的:从视网膜功能和形态相关性评价曲安奈德玻璃体注射治疗黄斑囊样水肿(CME)临床疗效。方法:选择29例29眼CME患者,进行玻璃体腔注射曲安奈德(IVTA)2 mg治疗,治疗前及治疗后1个月、3个月、6个月随访,进行最佳矫正视力(BCVA)、晶状体、眼内压(IOP)、OCT测量黄斑中心凹厚度(CFT)、多焦视网膜电图(mf-ERG)检查及FFA检查等。结果:治疗后患者视力明显提高,CFT明显降低,治疗前后差异有统计学意义(P<0.05)。mf-ERG:N1波潜伏期变化不大,P1波潜伏期及N1、P1波振幅密度有所改善,差异无统计学意义(P>0.05),N1、P1波潜伏期与CFT无相关性(P>0.05),N1、P1波振幅密度与CFT存在负相关性(P<0.05)。结论:mf-ERG联合OCT能更客观、更全面的评价IVTA治疗CME在黄斑功能和形态上的变化。 Objective:To evaluate the therapeutic effects of triamcinolone intravitreal injection in the treatment of cystoid macular edema(CME) through retinal function and morphological correlation.Methods:29 patients with 29 eyes of CME were selected.They were treated with 2 mg intravitreal injection of triamcinolone(IVTA),then followed up before the treatment and 1 months,3 months,6 months after the treatment,recorded their best corrected visual acuity(BCVA),lens,intraocular pressure(IOP),central macular foveal thickness(CFT) measured by OCT,multifocal electroretinogram(mf-ERG) examination and FFA etc.Results:After the treatment,the patient’s vision was increased,CFT decreased significantly,there were statistically significant differences before and after the treatment(P<0.05).Mf-ERG:N1 wave latency changed a little,the latency of P1 wave and amplitude density of N1,P1 wave were improved,however,there had no significant difference before and after the treatment(P>0.05),there was no significant correlation between latency of N1,P1 wave and CFT(P>0.05),the amplitude density of N1 and P1 wave has a negative correlation with CFT(P<0.05).Conclusion:mf-ERG combined with OCT can evaluate the change on macular function and morphology more objective and comprehensive,with IVTA in the treatment of CME.
