二尖瓣置换术(MVR)运用七氟醚处理对患者左心室舒张功能及TNF-α、BNP的影响研究

Influence research of sevoflurane treatment on the left ventricular diastolic function and TNF-alpha,BNP used in the mitral valve replacement(MVR)

目的:探讨二尖瓣置换术运用七氟醚处理对患者左心室舒张功能及TNF-α、BNP的影响。方法:收治择期行心肺流转下二尖瓣置换术患者64例,随机分为观察组34例,对照组30例,两组采取相同的麻醉方法,观察组在CPB结束后给予七氟醚吸入,比较两组患者麻醉诱导后不同时间点左心室舒张功能各指标,以及术后不同时间血浆TNF-α以及BNP浓度。结果:T3、T4、T5时间两组比较观察组E/E0显著低于对照组(P<0.05);T3、T4时间观察组E/Vp显著低于对照组(P<0.05);T5、T6、T7时间观察组TNF-α显著低于对照组(P<0.05);T6、T7时间观察组BNP显著低于对照组(P<0.05)。结论:MVR患者在CBP后运用七氟醚处理能够在一定程度上缓解患者的左心室舒张功能的减退,且具有保护心肌细胞的作用。

Objective:To investigate the influence research of sevoflurane treatment on the left ventricular diastolic function and TNF-alpha,BNP used in the mitral valve replacement(MVR).Methods:64 patients with undergoing Mitral valve replacement with cardiopulmonary circulation were randomly divided into the observation group with 34 cases,and the control group with 30 cases.The two groups took the same anesthesia method.The observation group were given sevoflurane inhalation after the CPB.We compared the indexes of left ventricular diastolic function,the plasma level of TNF-alpha and BNP concentration at the different time after operation at different time points after the induction of anesthesia of the two groups patients.Results:The E/E0 of the observation group was significantly lower than that the control group at the T3,T4,T5time(P<0.05);the E/Vp of the observation group was significantly lower than that of the control group at the T3,T4time(P<0.05);the TNF-alpha of the observation group was significantly lower than that of the control group at the T5,T6,T7time(P<0.05);the BNP of the observation group was significantly lower than that of the control group at the T6,T7time(P<0.05).Conclusion:Patients with MVR use sevoflurane treatment can alleviate on certain level of the left ventricular diastolic dysfunction after CBP,and can protect myocardial cells.
