社区糖尿病患者低血糖护理干预路径

The nursing intervention path for hypoglycemia of patients with diabetes mellitus at community

目的:探讨社区糖尿病患者低血糖护理干预路径,以实现医院-社区-家庭护理干预模式,促进多学科合作。方法:收治糖尿病患者100例,进行随访指导,研究糖尿病患者低血糖护理干预路径前后生活质量、糖尿病知识评分和临床治疗效果。结果:生活质量、糖尿病知识评分和临床治疗效果干预前后的差异均具有统计学意义(P<0.05)。结论:实施低血糖护理干预路径,能有效减少社区糖尿病患者低血糖事件的发生,提高患者的生活质量。

Objective:To explore the nursing intervention path for hypoglycemia of patients with diabetes mellitus at community,in order to realize the hospital-community-family nursing intervention mode to promote multidisciplinary cooperation.Methods:100 patients with diabetes were selected.They were given follow-up guidance,before and after the nursing intervention path for hypoglycemia,and we studied the quality of life of patients with diabetes,diabetes knowledge score and clinical therapeutic effect.Results:The difference of the quality of life,diabetes knowledge score and clinical therapeutic effect before and after the nursing intervention was statistically significant(P<0.05).Conclusion:The implementation of hypoglycemia nursing intervention path can effectively reduce the hypoglycemia of patients with diabetes mellitus at community,and improve the quality of life of patients.
