PFNA内固定治疗老年股骨转子间骨折的临床体会

Clinical experience of PFNA internal fixation for the treatment of femoral intertrochanteric fractures in the elderly

目的:研究分析股骨近端防旋刀片髓内钉(PFNA)内固定治疗老年股骨转子间骨折的临床效果。方法:收治老年股骨转子间骨折患者38例,给予PFNA内固定进行治疗,观察患者的Harris髋关节功能评分。结果:38例患者的手术切口顺利愈合,术后X片检查显示,骨折处对线良好。随访7~18个月,平均11个月,骨折均愈合,根据Harris髋关节功能评分标准,优26例,良10例,差2例,总优良率94.7%。结论:PFNA内固定治疗老年股骨转子间骨折疗效确切,X线透视下的微创手术,有利于软组织修复和骨折愈合,提高患者的生活质量及临床疗效,是治疗老年股骨转子间骨折的有效方法。

Objective:To study and analyze the clinical effect of proximal femoral nail antirotation(PFNA)internal fixation for the treatment of femoral intertrochanteric fractures in the elderly.Methods:38 elderly patients with femoral intertrochanteric fractures were selected.They were treated with PFNA internal fixation.We observed the patients'Harris score of hip joint function.Results:38 cases of patients with healed incision operation,postoperative X examination showed that the fracture was good on the line, follow up for 7~18 months,average 11 months,all fractures healed.According to the Harris hip score,26 cases were excellent,good in 10 cases,poor in 2 cases,the total excellent rate was 94.7%.Conclusion:The effect of PFNA internal fixation in the treatment of senile intertrochanteric fracture is curative,minimally invasive surgery under X-ray fluoroscopy,and help the soft tissue repair and healing,improve quality of life and clinical effect in patients.It is an effective method for the treatment of intertrochanteric fractures in the elderly.
