不同肺静脉造影方式对阵发性心房颤动环肺静脉隔离术主要指标的影响

Effect of different pulmonary vein angiography on main parameters of circumferential pulmonary vein isolation in patients with paroxysmal atrial fibrillation

目的探讨选择性和非选择性肺静脉造影对阵发性心房颤动环肺静脉隔离术主要指标的影响。方法入选2015年6月至2017年6月在厦门市心脏中心因阵发性心房颤动接受环肺静脉隔离的患者96例随机分入非选择性肺静脉造影组(45例),选择性肺静脉造影组(51例),分析两组患者肺静脉造影时间、X线曝光总时间、累计放射剂量,6个月的手术成功率。结果非选择性肺静脉造影组与选择性肺静脉造影组对比,造影时间短[(21.42±4.07)s vs (110.66±12.78)s,P<0.01],累计放射剂量少[(72.94±6.23)mGy vs (131.46±15.77)mGy,P<0.01],X线曝光时间差异无统计学意义[(23.43±6.98)min vs (25.24±6.13)min,P>0.05],6个月后手术成功率差异无统计学意义(75.56%vs 76.47%,P>0.05)。结论非选择性肺静脉造影能满足消融手术需要。

Objective To investigate the effect of selective and non-selective pulmonary vein angiography on the main parameters of circumferential pulmonary vein isolation(CPVI)for paroxysmal atrial fibrillation(PAF).Methods A total of 96 patients undergoing circumferential pulmonary vein isolation due to PAF in the heart center of Xiamen city between June 2015 and June 2017 were enrolled.They were randomly divided into non-selective pulmonary vein angiography group(45 cases)and selective pulmonary vein angiography group(51 cases).Pulmonary vein angiography time,total X-ray fluoroscopy time,and cumulative radiation dose,6-month success rate of procedure were recorded.Results Pulmonary vein angiography time was shorter[(21.42±4.07)s vs.(110.66±12.78)s,P<0.01]and cumulative radiation dose was less[(72.94±6.23)mGy vs.(131.46±15.77)mGy,P<0.01]in non-selective than in selective pulmonary vein angiography group.There were no statistical difference in total X-ray fluoroscopy time and 6-month success rate between the two groups.Conclusion Non-selective pulmonary vein angiography can meet the needs of ablation procedure.
