低剂量CT对无症状肺癌高危人群的筛查价值

Screening Value of Low-dose CT in High-risk Population of Asymptomatic Lung Cancer

目的:探讨低剂量CT在无症状肺癌高危人群早期筛查中作用与意义。方法:随机选取2017年3月~2018年3月在我院参加'城市癌症早诊早治项目'筛查的244例肺癌高危人群作为观察对象。采用随机数字表法将研究对象分为观察组和对照组,每组各122例。观察组采用低剂量CT扫描;对照组采用常规剂量CT扫描。记录主机自动生成的CTDIvol、DLP、总扫描时间,并计算相应的ED。对比两组肺结节检出率和图像质量评分。结果:两组平均图像质量评分对比,差异无统计学意义(P>0.05)。两组高危人群肺结节和早期肺癌检出率对比,差异均无统计学意义(P>0.05)。观察组CTDIvol、DLP、ED均明显低于对照组,总扫描时间也明显较对照组减少,差异有统计学意义(P<0.05)。结论:低剂量CT检查可以在较低剂量暴露条件下,保证图像质量以满足无症状肺癌高危人群的筛查要求。

Objective:To investigate the role and significance of low-dose CT in early screening of asymptomatic lung cancer at high risk.Methods:A total of 244 high-risk groups of lung cancer who were screened in the'Urban Cancer Early Diagnosis and Treatment Program'from March 2017 to March 2018 were randomly selected.The subjects were divided into observation group and control group by random number table method,with 122 cases in each group.The observation group was scanned with low-dose CT;the control group was scanned with conventional dose CT.Record the CTDIvol,DLP,total scan time automatically generated by the host,and calculate the corresponding ED.The lung nodule detection rate and image quality score were compared between the two groups.Results:There was no significant difference in the average image quality score between the two groups(P>0.05).There was no significant difference in the detection rate of lung nodules and early lung cancer between the two groups(P>0.05).The CTDIvol,DLP and ED of the observation group were significantly lower than the control group,and the total scan time was also significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Low-dose CT examination can ensure image quality under low-dose exposure conditions to meet the screening requirements of high-risk populations of asymptomatic lung cancer.
