腹腔镜辅助下胃癌D2根治术对进展期远端胃癌的治疗价值

Analysis of Curative Value of Laparoscopically assisted in D2 Re-section of Gastric Cancer

目的：研究腹腔镜辅助下胃癌D2根治术对进展期远端胃癌的治疗价值。方法选取2012年7月～2013年8月我院100例进展期远端胃癌患者，随机分为两组各50例。治疗组采用腹腔镜辅助下远端胃癌D2根治术，对照组行开腹远端胃癌D2根治术，并对两组患者的临床疗效进行评价。结果治疗组出血量、切口长度、术后止痛次数、并发症明显少于对照组，住院时间、肛门排气时间、下床活动时间明显短于对照组（P＜0.05）；治疗组手术时间长于对照组（P＜0.05）；两组平均肿瘤长度、清扫淋巴结数量无统计学差异(P＞0.05)。结论腹腔镜辅助下胃癌D2根治术对进展期远端胃癌，具有恢复快，出血少，创伤小等特点，值得临床推广应用。

Objective To study the clinical value of laparoscopic and D2 resection of gastric cancer. Methods 100 patients with gastric cancer underwent gastrointestinal surgical radical re-section of distal D2, from July 2012 to August 2013 in our hospital, were randomly divided into two groups, 50 cases of each group. The treatment group was used laparoscopic assisted distal gastric cancer radical surgery for D2 treatment, the control group were treated with open distal D2 radical gastrectomy. Than the effect of the two groups were compared. Results The amount of bleeding, the incision length, the average postoperative injection of analgesic drugs and complica-tion in treatment group, were significantly less than the open group. The hospitalization time, the first anal exhaust time and postoperative ambulation time, were significantly shorter than the con-trol group (P<0.05); The treatment group of the operation time was significantly longer than the control group(P<0.05); The differences of mean tumor length and number of lymph nodes were not statistically significant (P>0.05). Conclusion The operation of laparoscopically assisted gastrectomy distal D2 have the advantages of small operation wound, faster postoperative recovery, debridement thoroughly, high security and were worthy of further application in clinical practice.
