Angiojet抽吸导管在下肢深静脉血栓治疗中的应用

Application of Angiojet aspiration catheter in the treatment of lower extremity deep venous thrombosis

目的总结本中心使用Angiojet抽吸导管治疗下肢深静脉血栓的初步临床经验。方法汇总、分析2015年10月1日至2015年12月30日天津医院血管外科使用Angiojet血栓抽吸系统治疗6例下肢急性深静脉血栓形成(deep venous thrombosis,DVT)患者的临床资料。结果全部6例患者,手术技术成功率为100%,其中1例患者抽吸效果差,进一步行导管溶栓术,2例漂浮血栓患者漂浮血栓消失,其余患者在抽吸后可见顺畅静脉血流。1例患者出现术后血尿和肾功能损伤。结论本研究初步经验为:单独使用血栓抽吸术的适应人群为短段新鲜血栓、抗凝禁忌和漂浮血栓患者,在使用过程中应该严格控制抽吸时间,抽吸效果不佳的患者可以行导管溶栓术。

Objective To summarize the clinical experience in the treatment of deep venous thrombosis of lower extremity with Angio Jet Rheolytic Thrombectomy. Methods The clinical data of 6 patients with acute deep venous thrombosis of lower extremity treated by Angiojet thrombus aspiration system from October 1, 2015 to December 30, 2015 were collected and analyzed. Results All 6 cases of patients, surgical technique success rate was 100%, 1 patient performed further catheter directed thrombolysis, two cases of floating thrombus was disappeared, the rest of the patients could see smooth venous blood flow after procedure. Hematuria and renal disfunction occurred in 1 patients. Conclusion Our preliminary experience is using of thrombus aspiration alone is adapt to patients who suffered from fresh thrombus, contraindications to anticoagulation and floating thrombus. patients using process should be strictly controlled suction time, the patients whose the suction effect was poor should perform catheter directed thrombolysis
