利伐沙班预防骨科手术后深静脉血栓形成的疗效

Therapeutic effect of rivaroxaban on prevention of deep vein thrombosis after orthopedic surgery

目的探讨利伐沙班预防骨科手术后深静脉血栓形成(DVT)的疗效。方法选择2016年3月至2017年9月在青岛大学医疗集团莒县人民医院进行骨科大手术后的患者78例,随机分为观察组(n=39)和对照组(n=39)。观察组接受利伐沙班治疗,给予对照组静脉滴注低分子肝素纳治疗,均治疗1周。记录并比较2组患者骨科手术后DVT的发生率,比较2组患者干预前后的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)。结果观察组患者的DVT发生率(2.56%)显著低于对照组(15.38%),差异有统计学意义(P<0.05)。2组患者治疗前PT、APTT比较差异无统计学意义(P>0.05),观察组治疗后的PT、APTT与对照组比较差异有统计学意义(P<0.05)。结论利伐沙班能显著降低骨科手术后患者出现DVT的概率,提高患者的治疗满意度,值得临床推广研究。

Objective To investigate the efficacy of rivaroxaban for prevention of deep vein thrombosis after orthopedic surgery. Methods A total of 78 patients who underwent major orthopaedic surgery in our hospital from March 2016 to September 2017, were randomly divided into the observation group(n=39) and control group(n=39). The observation group was received rivaroxaban treatment, and the control group was received intravenous infusion of low molecular weight heparin treatment. All treatments lasted for 1 week. The incidence of deep vein thrombosis was recorded and compared between the two groups after orthopedic surgery. The prothrombin time(PT) and activated partial thromboplastin time(APTT) were compared before and after intervention. Results The incidence of deep vein thrombosis in the observation group(2.56%) was significantly lower than that in the control group(15.38%), and the difference was statistically significant(P<0.05). There were no significant differences in PT and APTT between the two groups before treatments(P>0.05). The PT and APTT values in the observation group were significantly better than those in the control group(P<0.05). Conclusion Rivaroxaban can significantly reduce the probability of deep vein thrombosis of patients after orthopedic surgery and improve their satisfaction. It is worthy of clinical promotion.
