红外线热成像仪评价组织坏死期血栓闭塞性脉管炎的治疗效果

Evaluation of the therapeutic effect of tissue necrosis thromboangiitis obliterans by infrared thermal imager

目的探讨应用红外线热成像仪评价组织坏死期血栓闭塞性脉管炎(LEN-TAO)治疗效果的可行性。方法对2017年1月至2018年5月解放军总医院已确诊LEN-TAO并进行介入治疗的16例患者进行病足红外线热成像仪检查,分析患侧脚掌各位点术前及术后3天温度变化的统计学差异。结果手术成功率为100%,所有患者症状改善,围术期无死亡和并发症。术后3天患侧脚掌中心温度、病变中心温度、病变周边温度明显升高(P均<0.05),患侧临近足趾温度升高,差异有统计学意义(P=0.04)、病变中心与临近足趾温度差绝对值明显缩小,差异有统计学意义(P=0.02),对侧相同位置足趾温度变化及病变足趾各位点与脚掌中心温度变化差值绝对值无统计学差异。结论红外线热成像仪检查是评估LEN-TAO治疗效果的一种客观有效的方法,以病变中心、病变周边、病变中心与临近足趾温度差绝对值温度变化来评价LEN-TAO治疗效果的短期评价较好,中长期评价待进一步随访研究。

Objective To investigate the feasibility of using infrared thermal imaging to evaluate the therapeutic effect of Necrosis of thromboangiitis obliterans of lower extremities(LEN-TAO).Methods From January 2017 to May 2018,16 patients with confirmed LEN-TAO and interventional treatment in the General Hospital of the People's Liberation Army were examined by infrared thermal imaging.The statistics of preoperative and postoperative temperature changes were analyzed.Learn the difference.Results The success rate was 100%,all patients had improved symptoms,and there were no deaths and complications during the perioperative period.At 3 days postoperatively,the center temperature of the affected foot,the temperature of the lesion center,and the temperature around the lesion were significantly increased(P<0.05),and the temperature of the affected side was increased near the toe.The difference was statistically significant(P=0.04).The absolute value of the temperature difference between the toes and the toes was significantly reduced,and the difference was statistically significant(P=0.02).There was no statistically significant difference in the temperature of the toe temperature at the same position on the contralateral side and the absolute difference in the temperature difference between the point of the toe and the center of the foot.Conclusion Infrared thermography examination is an objective and effective method to evaluate the therapeutic effect of thromboangiitis obliterans in tissue necrosis.It is strongly recommended to change the absolute temperature of the lesion center,lesion periphery,lesion center and adjacent toe temperature difference.To evaluate the short-term evaluation of the efficacy of LEN-TAO treatment,to adjust the treatment plan in time,and to evaluate the treatment in the medium and long term.
