摘要
目的探讨机械血栓清除术治疗急性下肢深静脉血栓(DVT)的疗效。方法选择湖北省荆州市第二人民医院2017年1月至2019年3月收治的60例急性下肢DVT患者,按照治疗方法不同分为对照组30例接受置管溶栓治疗,研究组30例接受机械血栓清除术。统计两组总有效率和并发症发生率;下肢周径差值、静脉通畅评分;采用视觉模拟评分(VAS)对症状进行评分。结果研究组的临床总有效率为96.67%高于对照组73.33%,差异有统计学意义(P<0.05)。治疗后,研究组患侧—健侧大腿、小腿周径差值和静脉通畅及VAS评分均小于对照组,差异有统计学意义(P<0.05)。研究组和对照组并发症发生率分别为3.33%和6.67%,两组比较差异无统计学意义(P>0.05)。结论机械血栓清除术用于急性下肢DVT治疗中具有良好的临床疗效,可有效缓解患者下肢消肿,恢复静脉通畅。
Objective To investigate the effect of mechanical thrombectomy on acute deep venous thrombosis(DVT)of lower extremities.Method Sixty patients with acute lower extremities DVT admitted to The Second Hospital of Jingzhou from January 2017 to March 2019.According to different treatment methods,30 patients in the control group were treated with catheter thrombolysis and 30 patients in the study group were treated with mechanical thrombectomy.The total effective rate and the incidence of complications,the difference of lower limb circumference and the score of venous patency were counted,and the symptoms were scored by visual analogue scale(VAS).Result The total clinical effective rate of study group was 96.67%higher than that of the control group 73.33%,the difference was statistically significant(P<0.05).After treatment,the difference of thigh and calf circumference,venous patency and VAS scores of the affected side and healthy side in the study group were lower than those of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group and the control group were 3.33%and 6.67%,respectively.There was no significant difference between the two groups(P>0.05).Conclusion Mechanical thrombectomy has a good clinical effect in the treatment of acute DVT of lower limbs.It can effectively relieve the swelling of lower limbs and restore venous patency.
