CT血管造影联合CT灌注在缺血性脑卒中的临床应用

Clinical application of CT angiography combined with CT perfusion in ischemic stroke

目的探讨在缺血性脑卒中患者中应用计算机断层扫描(CT)血管造影联合CT灌注的临床价值。方法从鄂尔多斯市中心医院放射科择取88例缺血性脑卒中患者,所选患者均行CT血管造影联合CT灌注检测,对检出结果进行分析。结果经CT灌注检查,发现74例异常灌注;CT血管造影检查,发现颈部动脉狭窄和颅内动脉狭窄患者有66例;CT血管造影联合CT灌注共检出79例异常,联合诊断检出率高于单独诊断,差异有统计学意义(P<0.05)。与镜像区比较,感兴趣区病变脑组织脑血流量和脑血容量也下降,平均通过时间延长,差异有统计学意义(P<0.05)。结论在缺血性脑卒中患者中应用CT血管造影联合CT灌注的临床价值显著,可以反映病变脑组织血流动力学变化,判别颈部血管狭窄和颅内血管狭窄,为缺血性脑卒中临床诊断提供重要依据。

Objective To investigate the clinical value of computed tomography(CT)angiography combined with CT perfusion in patients with ischemic stroke.Method 88 patients with ischemic stroke were selected from the Ordos Central Hospital.All patients were examined by CT angiography combined with CT perfusion,and the results were analyzed.Result CT perfusion showed abnormal perfusion in 74 cases,CT angiography showed carotid artery stenosis and intracranial artery stenosis in 66 cases,and CT angiography combined with CT perfusion detected 79 cases of abnormal perfusion,the detection rate of combined diagnosis was higher than that of single diagnosis,the difference was statistically significant(P<0.05).Compared with the mirror area,the cerebral blood flow and volume in the lesions of the region of interest also decreased,and the average transit time was prolonged,with statistical significance(P<0.05).Conclusion The clinical value of CT angiography combined with CT perfusion in patients with ischemic stroke is significant.It can reflect the hemodynamic changes of lesion brain tissue,distinguish cervical and intracranial vascular stenosis,and provide important basis for clinical diagnosis of ischemic stroke.
