电话督导弹力袜治疗在下肢静脉曲张患者腔内激光闭合术后随访中的临床意义

The effects of telephone guided elastic stocking for patients with varicose veins after endovenous laser ablation

目的探讨电话督导弹力袜治疗对下肢静脉曲张患者激光闭合术后恢复的影响。方法2013年1月至2013年12月,采用腔内激光闭合术治疗的246例静脉曲张患者,患者随机分为两组:电话督导弹力袜治疗127例(实验组)及出院宣教弹力袜治疗的119例(对照组)。术后随访比较两组患者弹力袜治疗的依从性、静脉曲张复发率及相关并发症的影响。结果实验组和对照组弹力袜治疗的依从性分别为80.3%及51.3%(P<0.01),术后1周实验组患者术肢疼痛、肿胀等严重程度要低于对照组(P<0.05),1个月及1年两组比较差异无统计学意义(P>0.05)。平均随访13个月,复发率分别为1.6%及1.7%(P=0.66)。结论电话督导弹力袜治疗可以显著提高静脉曲张患者激光闭合术后依从性,早期可以改善患者静脉疾病严重度和生活质量,但对术后静脉曲张复发率无明显影响。

Objective To discuss the effectiveness for patients with varicose veins after endovenous laser ablation treated by elastic stocking with telephone guidance during the following up.Methods A prospective analysis of patients with varicose veins treated by endovenous laser ablation was conducted at a single constitution from January 2013 to December 2013.All patients were divided into telephone-guided elastic stocking(case group)and discharge education guided elastic stocking(control group)according to randomized criteria after surgery.Primary end points were patients ' compliance,recurrence of varicose veins,and the severity of venous disorders.Results There were totally 127 patients and 119 patients included in case group and control group.The rate of patients complied with elastic stocking were 80.3% and 51.3%respectively(P<0.01).The severity of venous disorders in case group was better than in control group in the first week after EVLA.Results were similar in a longer time.The recurrence of varicose veins was 1.6% and1.7% between the groups during mean 13 months following up postoperatively.Conclusions Telephone-guided elastic stocking could increase the patients ' compliance to compression therapy,and improve the severity of venous disorders in early term after EVLA,but no beneficial for the varicose veins recurrence.
