唇裂修复与鼻部整形的联合应用在矫形治疗效果观察

The effect of rhinoplasty combined with cleft lip repair on the effective study of orthopedic treatment

目的观察单侧完全性唇裂患儿应用鼻整形联合鼻撑矫形治疗的效果,旨在为完全性唇裂患儿鼻畸形提供更有效整形方式。方法收集本院2014年2月-2018年2月收治的58例单侧完全性唇裂患儿临床资料进行分析,按入院先后顺序分为对照组(给予改良Millard法唇裂修复术)和观察组(给予鼻整形联合鼻撑矫形术)各29例,比较两组患儿术后左右鼻部相关测量指标非对称率及鼻尖偏离度情况,并评价治疗效果。结果观察组患儿术后左右鼻孔高度、鼻孔宽度、鼻翼长度、鼻基部长度、鼻偏曲度非对称率均显著低于对照组,经组间成组t检验(t=5.2416、5.6631、5.8871、4.5939、6.3071,P <0.01)。而观察组患儿术后左右鼻翼口角距、鼻翼唇峰距非对称率虽高于对照组,但经组间成组t检验(t=1.0808、1.3985,P> 0.05)。观察组患儿术后鼻尖偏离度明显<对照组,经t检验(t=10.0951,P <0.01)。结论单侧完全性唇裂患儿实施鼻整形联合鼻撑矫形修复术治疗效果显著,不仅改善患儿鼻部形态美观,也从一定程度上促进口腔颌面部向正常发育发展,值得临床推广运用。

Objective To observe the effect of rhinoplasty combined with rhinoplasty for the treatment of unilateral complete cleft lip in children,in order to provide a more effective shaping for nasal deformity in children with complete cleft lip.Methods The clinical data of 58 children with unilateral complete cleft lip in our hospital from February 2014 to February 2018 were retrospectively analyzed.According to the order of admission,they were divided into control group(given modified Millard cleft lip repair)and observation Group(nose remodeling combined with nasal prosthesis orthodontics)was respectively performed in 29 patients.The relative asymmetry rate and nose deviation of the left and right nose between the two groups were compared,and the treatment effect after surgery was evaluated.Results The left and right nostril height,nostril width,alar length,nasal base length,and nasal asymmetry asymmetry rate in the observation group were significantly lower than the control group.The intergroup by t test was performed(t=5.2416,5.6631,5.8871,4.5939,6.3071,P<0.01).In the observation group,the asymmetrical rate of left and right alar flaring angle and alar wing pitch were higher than those in the control group,t test results were(t=1.0808,1.3985,P>0.05).The degree of postoperative nasal tip deviation in the observation group was significantly smaller than the control group and by t test(t=10.0951,P<0.01).Conclusion The treatment of unilateral complete cleft lip with rhinoplasty combined with orthopaedic prosthesis has significant therapeutic effect.It not only improves the appearance of the nasal part of the children,but also promotes the normal development of the oral and maxillofacial parts to a certain extent.It is worthy of clinical application.
